Required Documentation For Payment/Request Procedure. An Employee seeking educational benefits must notify the Employer by October 1st of the year before which benefits must be budgeted for. An employee claiming this benefit must submit evidence to the Employer that the charges and textbooks were required for a course in one (1) of the above described programs. A paid receipt shall also be submitted showing payment in full for the charges and evidence that the employee has successfully completed the course for which the charges were required. The Employer shall not reimburse the employee for charges for which the employee is entitled to reimbursement from any other entity. In addition, the employee must provide the Fire Chief advance written notice of the employee’s intent to enroll. Failure to provide proper written notice shall relieve the Employer of its obligation to reimburse the employee.

  • Required Documentation The AGENCY is responsible for ensuring that all documents required by this Contract are current and available for the COUNTY’S review upon request. These documents may include, but are not limited to, certificate(s) of insurance, job descriptions and background check confirmations of staff.

  • Request for Payment Payment to the Grantee shall be due 30 calendar days following receipt by the City of the Grantee’s fully and accurately completed payment request, using the City’s contract management system. The payment request must be submitted to the City no later than 11:59 p.m. Central Standard Time 25 calendar days following the end of the month covered by the payment request. If the 25th calendar day falls on a weekend or holiday, as outlined in Section 8.24, the deadline to submit the payment request is extended to no later than 11:59 p.m. Central Standard Time of the 1st weekday immediately following the weekend or holiday.
