Return of Device. Customers must return the Device to us if 1) you cancel services and 2) you return the Device to us within fourteen (14) days of the date you disconnected Service. Advanced Stream Broadband will also waive our disconnection fee subject to the following: Service is disconnected within the first thirty (30) days following the activation of the Service, activation occurs when you place your order for service; We receive the Device in its original condition, reasonable wear and tear excluded; You return original proof of purchase with the Device, together with the original packaging, all parts, accessories, and documentation; and You must pay all costs of returning the Device back to us. If you receive a Device(s) that is visibly damaged, you must contact our customer care department immediately at 253-627-8000 or xxxxxx@xxxxxxxxxxxxxx.xxx.

Return of Device. The Device must be returned to VoiceLift within fourteen (14) days of the termination of Service to receive a credit for the $39.99 disconnect fee (refer to section 4.6 of this document regarding termination fees), provided: (i) you have retained, and return along with the Device, proof of purchase and original packaging; (ii) contents are undamaged and in original condition, reasonable wear and tear excluded; (iii) all parts, accessories, documentation and packaging materials are returned; and (iv) equipment is returned with a valid return authorization number obtained from VoiceLift’s customer care department. You are responsible for the cost and risk of return shipping of equipment. If you receive cartons and/or Devices that are visibly damaged, you must note the damage on the carrier's freight xxxx or receipt and keep a copy. In such event, you must keep the original carton, all packing materials and parts intact in the same condition in which they were received from the carrier and contact VoiceLift's customer care department immediately. Return of Device. This device is being loaned for the student’s use during remote learning. This device is the property of the NYCDOE and contains a tracking device so that the device can be located.
