Schedule of Eligibility. A. ELIGIBLE PERSON 1. An Eligible Person is defined as a regular full−time Employee of Tulsa County or an eligible entity, who regularly or normally works at least the number of hours in the normal work week set by the Employer (but not less than thirty [30] hours) at the Employer’s place of business or such other place or places as required by the Employer and is compensated by the Employer for personal services rendered the Employer. 2. The date the Eligible Person becomes eligible for coverage is the first of the month following one (1) month of continuous, full−time employment. 3. A person may not be simultaneously covered under the Plan as both an Employee and a Dependent. 4. The definition of Eligible Person shall not include: a. Employees of Tulsa County TCPFA, Tulsa County Drainage District #12, Tulsa Area Emergency Management Authority (TAEMA), and Tulsa County Public Defender Office; or b. COBRA participants previously eligible under Tulsa County TCPFA, Tulsa County Drainage District #12, Tulsa Area Emergency Management Authority (TAEMA), and Tulsa County Public Defender Office; or c. Retired Tulsa County employees.

Schedule of Eligibility. A. Eligibility 1. Subscriber. To be eligible to enroll as a Subscriber, an individual must be: a. an employee, who has satisfied any criteria designated by us, has satisfied any Eligibility Waiting Period required by the Group, and who is working the number of hours designated by the Company in the Application for Group Coverage. b. a retiree who satisfies any criteria designated by Us, and if shown as covered in this Group’s Benefit Plan Schedule of Vision Benefits. c. an elected official who satisfies any criteria designated by Us, and if shown as covered in this Group’s Benefit Plan Schedule of Vision Benefits.

  • Schedule of Services Consultant shall perform the Services in a prompt and timely manner in accordance with the Schedule of Services set forth in Exhibit "B" attached hereto and incorporated herein by reference. Consultant represents that it has the professional and technical personnel required to perform the Services expeditiously. Upon request of City, Consultant shall provide a more detailed schedule of anticipated performance to meet the Schedule of Services.

  • Student Eligibility The LEA and POSTSECONDARY INSTITUTION shall qualify and advise candidates for dual credit from the pool of eligible high school students. A candidate for dual credit is eligible for consideration for fall, spring, and summer semesters if he or she: a. is enrolled during the fall and spring in a LEA in one-half or more of the minimum course requirements approved by PED for public school students under its jurisdiction or by being in physical attendance at a bureau of Indian education-funded high school at least three documented contact hours per day pursuant to 25 CFR 39.211(c); b. obtains permission from the LEA representative (in consultation with the student’s individualized education program team, as needed), the student’s parent or guardian if the student is under 18 years old, and POSTSECONDARY INSTITUTION representative prior to enrolling in a dual credit course; and c. meets POSTSECONDARY INSTITUTION requirements to enroll as a dual credit student.

  • Continuing Eligibility To continue health benefits, a permanent intermittent employee must be credited with a minimum of 480 paid hours in a control period or 960 paid hours in two consecutive control periods.

  • Form S-3 Eligibility The Company is eligible to register the resale of the Securities for resale by the Purchaser on Form S-3 promulgated under the Securities Act.

  • Certification of eligibility a. By entering into this contract, the contractor certifies that neither it (nor he or she) nor any person or firm who has an interest in the contractor's firm is a person or firm ineligible to be awarded Government contracts by virtue of section 3(a) of the Xxxxx-Xxxxx Act or 29 CFR 5.12(a)(1). b. No part of this contract shall be subcontracted to any person or firm ineligible for award of a Government contract by virtue of section 3(a) of the Xxxxx-Xxxxx Act or 29 CFR 5.12(a)(1).
