Service Reporting. Grantee shall report service delivery data via encounters following the procedures and instructions, including all required data fields and values in the current version of the HHSC Community Mental Health Service Array. The HHSC Community Mental Health Service Array is incorporated by reference and is found within the General Warehouse Information, Specifications subfolder using the following link: xxxxx://xxxx0xxxxx0.xxxx.xxxxx.xxxxx.xx.xx/DataWarehousePage/. Grantee can find encounter reporting schedule in Information Item S, incorporated by reference and posted at: xxxxx://xxx.xxx.xxxxx.xxx/doing-business-hhs/provider-portals/behavioral-health- services-providers/behavioral-health-provider-resources/community-mental-health- contracts.
Service Reporting. Upon Named Entity’s written request, the Vendor, shall provide a report that includes the following national platform data for the prior month: SAVNS system down time, including the SAVNS core system, interface services, and notification interfaces.

Service Reporting. 2.5.1 During the Term the Provider shall, with respect to each Service for which the Service Schedule requires a certain Service Level, provide to the Recipient, as part of its monthly performance reports a set of hard copy and soft copy reports to verify the Provider’s performance and compliance with the Service Levels.
2.5.2 The Parties will review the Provider’s performance against the Service Levels for the previous reporting period.
Service Reporting. 2.5.1 From the Service Commencement Date and thereafter until the Scheduled End Date, each Provider shall, with respect to each Service for which the Service Schedule requires a certain Service Level:
(i) record the actual service level achieved for such Service in each calendar month against the relevant Service Level; and
(ii) on a quarterly basis (or such alternate frequency if so specified in the relevant Service Schedule) report its performance in respect of the applicable Service Level.
2.5.2 At the TSA Committee, the Parties will review each Provider’s performance against the Service Levels for the previous reporting period. In the event that a Provider has failed to meet any Service Level, that Provider shall present an appropriate action plan detailing steps to remedy the situation.
Service Reporting. 2.5.1 From the date hereof and thereafter until the earlier of the Scheduled MSA End Date or applicable Service Schedule Expiry Date, the Provider shall, with respect to each Service for which the Service Schedule requires a certain Service Level, provide to the Recipient, as part of its monthly performance reports a set of hard copy and soft copy reports to verify the Provider’s performance and compliance with the Service Levels and record the occurrence of any Criticality 1 Incidents.
2.5.2 The Parties will review the Provider’s performance against the Service Levels for the previous reporting period.
Service Reporting. Aspect shall maintain a complete record of all repair activities performed on any Aspect products received for repair, and will provide HP with a monthly report on all service actions including failure and repair statistics at a sub- assembly levelas laid down in Exhibit F (Service/Repair). Service Reports for each product shall be sent electronically by email to responsible procurement and technical marketing engineer (Contact Persons shown in Exhibit C, Contact Persons / Addresses). Rootcause analysis is to be performed and reported by Aspect in case of abnormal failures, incidents and malfunctions.
Service Reporting. Subcontractor shall report to TRANSBLUE the beginning and completion of each authorized service performed within 5 minutes of service start and completion.
