STAFFINfi. 22 17.1 CONTRACTOR shall provide the following described staff positions 23 and shall submit, in a format provided and/or approved by ADMINISTRATOR, 24 monthly staffing reports to ADMINISTRATOR. Staffing Reports shall report 25 actual staff hours worked by position, and shall include the position title 26 and monthly salary and benefits. CONTRACTOR shall recruit, train and 27 supervise Care Coordinator(s), Wraparound Supervisor(s), Parent Partner(s) and 28 Youth Partner(s) in accordance with standards developed by Wraparound OC for 1 provision of Wraparound. 2 17.2 CONTRACTOR shall notify ADMINISTRATOR within twenty-four (24) 3 standard business hours when a Care Coordinator, Wraparound Supervisor, Parent 4 Partner or Youth Partner is hired, providing the name and date hired for each 5 employee, as well as qualifications and copy(ies) of diploma(s). 6 17.3 CONTRACTOR shall, for each employee, maintain an employee file 7 that contains the Care Coordinator’s, Wraparound Supervisor’s, Parent 8 Partner’s, or Youth Partner’s resume and proof of qualifications. Upon 9 reasonable notice, CONTRACTOR shall provide ADMINISTRATOR access to employee 10 files to audit for compliance with this requirement. 11 17.4 CONTRACTOR shall comply with all CDSS Community Care Licensing 12 Division regulations for firoup Home Service Providers regarding background 13 checks on all potential employees.

  • Staffing There shall be a clinician employed by the outside contractor for EAP Services who will be on-site a minimum of 20 hours a week. The clinician shall report directly to the outside contractor, Peer Assistance Oversight Committee and the MIF liaison. There shall be three full-time Peer Assistants reporting to the outside contractor.

  • Surgery Services This plan covers surgery services to treat a disease or injury when: • the operation is not experimental or investigational, or cosmetic in nature; • the operation is being performed at the appropriate place of service; and • the physician is licensed to perform the surgery. This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia.

  • Pharmacy Services The Contractor shall establish a network of pharmacies. The Contractor or its PBM must provide at least two (2) pharmacy providers within thirty (30) miles or thirty (30) minutes from a member’s residence in each county, as well as at least two (2) durable medical equipment providers in each county or contiguous county.

  • Radiation Therapy/Chemotherapy Services This plan covers chemotherapy and radiation services. This plan covers respiratory therapy services. When respiratory services are provided in your home, as part of a home care program, durable medical equipment, supplies, and oxygen are covered as a durable medical equipment service.
