STAFFINfi. 11 17.1 CONTRACTOR shall provide the following described staff positions 12 and shall submit, in a format provided and/or approved by ADMINISTRATOR, 13 monthly staffing reports to ADMINISTRATOR. Staffing Reports shall report 14 actual staff hours worked by position, and shall include the position title 15 and monthly salary and benefits. CONTRACTOR shall recruit, train and 16 supervise Care Coordinator(s), Wraparound Supervisor(s), Parent Partner(s) and 17 Youth Partner(s) in accordance with standards developed by Wraparound OC for 18 provision of Wraparound. 19 17.2 CONTRACTOR shall notify ADMINISTRATOR within twenty-four (24) 20 standard business hours when a Care Coordinator, Wraparound Supervisor, Parent 21 Partner or Youth Partner is hired, providing the name and date hired for each 22 employee, as well as qualifications and copy(ies) of diploma(s). 23 17.3 CONTRACTOR shall, for each employee, maintain an employee file 24 that contains the Care Coordinator’s, Wraparound Supervisor’s, Parent 25 Partner’s, or Youth Partner’s resume and proof of qualifications. Upon 26 reasonable notice, CONTRACTOR shall provide ADMINISTRATOR access to employee 27 files to audit for compliance with this requirement. 28 17.4 CONTRACTOR shall comply with all CDSS Community Care Licensing 1 Division regulations for firoup Home Service Providers regarding background 2 checks on all potential employees.

  • Staffing There shall be a clinician employed by the outside contractor for EAP Services who will be on-site a minimum of 20 hours a week. The clinician shall report directly to the outside contractor, Peer Assistance Oversight Committee and the MIF liaison. There shall be three full-time Peer Assistants reporting to the outside contractor.

  • Surgery Services This plan covers surgery services to treat a disease or injury when: • the operation is not experimental or investigational, or cosmetic in nature; • the operation is being performed at the appropriate place of service; and • the physician is licensed to perform the surgery. This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia.

  • Orthodontics We Cover orthodontics used to help restore oral structures to health and function and to treat serious medical conditions such as: cleft palate and cleft lip; maxillary/mandibular micrognathia (underdeveloped upper or lower jaw); extreme mandibular prognathism; severe asymmetry (craniofacial anomalies); ankylosis of the temporomandibular joint; and other significant skeletal dysplasias.

  • Enterprise Information Management Standards Performing Agency shall conform to HHS standards for data management as described by the policies of the HHS Chief Data and Analytics Officer. These include, but are not limited to, standards for documentation and communication of data models, metadata, and other data definition methods that are required by HHS for ongoing data governance, strategic portfolio analysis, interoperability planning, and valuation of HHS System data assets.

  • Pharmacy Services The Contractor shall establish a network of pharmacies. The Contractor or its PBM must provide at least two (2) pharmacy providers within thirty (30) miles or thirty (30) minutes from a member’s residence in each county, as well as at least two (2) durable medical equipment providers in each county or contiguous county.

  • Professional Development and Training The Company shall pay for or reimburse the Executive for any reasonable professional development or training.

  • Radiation Therapy/Chemotherapy Services This plan covers chemotherapy and radiation services. This plan covers respiratory therapy services. When respiratory services are provided in your home, as part of a home care program, durable medical equipment, supplies, and oxygen are covered as a durable medical equipment service.

  • Medical Care The Parents must comply with the School Medical Officer's recommendations which may include a reasonable decision to release the Pupil home or to her education guardian when she is unwell.
