The CCR involves consultation with Network Providers and shall include, at a minimum, a statement of presenting problems including diagnosis, justification for extended services, a brief treatment plan including the number of additional requested services to resolve the problem, treatment goals, as well as information relevant to the specific diagnosis, mental status, symptomatology, functional impairment, and a description of linkages to other community resources and support groups. The CCR also may involve rescreening the beneficiary which, if applicable, shall include, at a minimum, determination of appropriate level of care, functional limitations and treatment barriers, service verification, identification of unmet resource needs and self-report measure of treatment effectiveness and satisfaction.

  • JOINT CONSULTATION 21.01 The parties acknowledge the mutual benefits to be derived from joint consultation and are prepared to enter into discussion aimed at the development and introduction of appropriate machinery for the purpose of providing joint consultation on matters of common interest.

  • Joint Consultation Committee 8.01 On the request of either party, the parties must meet at least once every four (4) months, for the purpose of discussing issues relating to the workplace that affect the parties or any employee bound by the Agreement.
