Case Management Services. Services that develop case plans for delivering community services to PATH eligible Consumers. The case plans should be developed in partnership with Consumers who receive PATH services to coordinate the assessment, treatment, housing and/or care of Consumers, tailored to Consumer needs and preferences. Case Managers assist the Consumer in accessing needed services, coordinate the delivery of services in accordance with the case plan, and follow-up and monitor progress. Activities may include financial planning, access to entitlement assistance, representative payee services, etc. Case Management Services include:
a. Preparing a plan for the provision of Community Mental Health Services to the eligible homeless Consumer involved, and reviewing such plan not less than once every three months;
b. Providing assistance in obtaining and coordinating social and maintenance services for the eligible homeless Consumers, including services relating to daily living activities, personal financial planning, transportation services, and Habilitation and Rehabilitation Services, pre-vocation and vocational services, and housing;
c. Providing assistance to the eligible homeless Consumers in obtaining income support services, including housing assistance, food stamps, and supplemental security income benefits;
d. Referring the eligible homeless Consumer for such other services as may be appropriate; and
e. Providing representative payee services in accordance with Section 1631(a) (2) of the Social Security Act if the eligible homeless Consumer is receiving aid under Title XVI of such act and if the Respondent is designated by the Secretary of the US Department of Health and Human Services, to provide such services.
Case Management Services. Services that assist PROVIDER in designing and implementing strategies for obtaining services and support that are goal oriented and individualized and that assist consumers with access to needed health services, financial assistance, housing, employment, education, social services and other services. PROVIDER must comply with MSHN Case Management Policy and in accordance with the MSHN-SUDSP Manual.
Case Management Services. Case management services shall include follow-up and referral of identified health care, nutritional, developmental, and psychosocial and safety needs.
Case Management Services. Case Management services will be provided to the persons who are homeless / chronically homeless who utilize this low-barrier shelter and service center. The case managers will aid in triaging participants to services that best meet their needs, and then link them to those area services. The goal of low-barrier services is to provide progressive engagement to chronically homeless who are challenging to serve and reluctant to seek assistance due to mental health challenges, substance use, and chronic health conditions. Once linked to existing services within the County, participants will be on a path that leads towards permanent supportive housing, alternative stable housing, and stability. Case managers may also help divert people from shelter through family reunification and other means. Through linkages and diversion Case Managers will work to decrease length of stay at the OC Courtyard and increase stability. This project will align with the Orange County Continuum of Care (CoC) and coordinated entry system. Staff will receive Orange County CoC / CES / or other training offered in best practices and local CoC process.
Case Management Services. Contractor shall be responsible for incorporating the CalWORKs 2.0 principles in order to assist ACSSA with implementing a more supportive level of case management services for the most disadvantaged groups of participants who come to ACSSA for services. Below is a listing of the major qualities associated with the CalWORKs 2.0 approach that the Contractor shall incorporate into their case management approach:
i. Open-minded. Judgment or criticism can act as barriers to communication, so it is important to maintain openness during the interview process. If interviewees perceive that they are being judged or evaluated, then they are less likely to openly share their opinions.
Case Management Services. Contractor shall provide Case Management services to the persons experiencing homelessness who are engaged. Case Management Services. 3.4.3.2.1 The Contractor shall provide case management services as outlined in Section 3.7 of this Contract to DSHP Plus LTSS members.
Case Management Services a. Contractor shall provide strength-based case management services (“Case Management Services”). Such Case Management Services shall focus on client needs, strengths and choices, and shall involve the client in service planning and implementation. The goal of Case Management Services is to help clients take charge of their own lives through informed decision making. Case Management Services shall assist the client in acquiring skills and support systems needed to function successfully in environments where they choose to live, learn, work and socialize.
b. Contractor shall staff two Case Management Services programs, 1) Intensive Case Management (REACH) and 2) New Ventures Case Management (New Ventures/Tehanan) which includes the Wellness and Recovery Action Partnership Program (WRAPP) (as each is defined below). Staffing for the two Case Management Services programs operated by Contractor shall be multi-disciplinary and reflect the cultural, linguistic, ethnic, age, gender, sexual orientation, and other social characteristics of the community served. Staff for both Case Management Services shall include both licensed and non-licensed individuals who are trained to provide mental health services and welcome and address co-occurring disorders.
Case Management Services. Case management services contribute to the coordination and integration of care delivery through the ongoing monitoring of services as prescribed in each enrollee’s care plan. Case management services facilitate enrollees gaining access to needed medical, social, transportation, and educational services regardless of the funding source for the services. The Contractor shall provide this service directly and the ratio of enrollees to case managers shall be appropriate to support the needs of the enrollees. The Contractor shall assist enrollees to ensure continuous financial and medical eligibility in the program as part of the case management responsibilities.
Case Management Services. If the Project Sponsor is using Grant Funds to provide case management services, the Project Sponsor shall ensure that qualified facility- based agencies are contacted to provide individuals with housing prior to utilizing lodging facilities, such as hotels, motels, and other similar establishments, to provide housing. Proof of such attempts shall be submitted with the request for reimbursement as described in Article III, Section 3 herein. Project Sponsors providing case management shall also comply with the attached Scope of Services.
