The Participant and/or designee agree to complete DHS-sponsored mandatory training prior to the submission of this Agreement and at any other time required by DHS. The Participant and/or designee agree to complete the online CCDF Participant Agreement refresher course once every twelve (12) months. If training is not completed by the training due date, payments will be suspended until training is completed and/or a corrective action plan implemented. In the event the mandated training is not completed within sixty (60) days after the training due date or a corrective action plan implemented, this Agreement will be cancelled.

The Participant and/or designee agree to complete DHS-sponsored mandatory training prior to the submission of this Agreement and at any other time required by DHS. The Participant and/or designee agree to complete refresher training once every twelve (12) months. If training is not completed by the training due date, payments will be suspended until training is completed and/or a corrective action plan implemented. In the event that the mandated training is not completed within sixty (60) days after the training due date or a corrective action plan implemented, this Agreement will be cancelled. If an Agreement is cancelled in this way, a new Participant Agreement will not be accepted for sixty (60) days after the cancellation date. The Department at its discretion may mandate additional training. Examples of when additional mandated training may be required include, but are not limited to, Participants subject to adverse action, billing discrepancies, changes in key staff, etc.

  • Referral to the Committee a) Prior to referral to the Committee, the matter shall be brought to the attention of the other local party.
