Vehicular homicide. Unauthorized leave of a mentally ill offender or a sexual or violent offender from a mental health facility, secure Community Transition Facilities (i.e. Evaluation and Treatment Centers, Crisis Stabilization Units, Secure Detox Units, and Triage Facilities) that accept involuntary admissions. Any event involving an Enrollee that has attracted or is likely to attract media attention.

The report shall include: 7.4.2.1 The date the Contractor becomes aware of the incident; 7.4.2.2 The date and time of the incident; 7.4.2.3 A description of the incident; 7.4.2.4 The name of the facility where the incident occurred, or a description of the incident location; 7.4.2.5 The name(s) and age(s) of Individuals involved in the incident; 7.4.2.6 The name(s) and title(s) of facility personnel or other staff involved; 7.4.2.7 The name(s) and relationship(s), if known, of other persons involved and the nature and degree of their involvement; 7.4.2.8 The Individual’s location at the time of the report if known (i.e. home, jail, hospital, unknown, etc.) or actions taken by the Contractor to locate the Individual; 7.4.2.9 Actions planned or taken by the Contractor to minimize harm resulting from the incident; and 7.4.2.10 Any legally required notifications made by the Contractor. 7.4.3 The Contractor shall report Critical Incidents using the HCA Incident Reporting System at xxxxx://xxxxxxxx.xx.xxx/hca/ics/ if the system is unavailable the Contractor shall report Critical Incidents to XXXXXXXX@xxx.xx.xxx. 7.4.4 The Contractor shall submit follow-up reports using the Incident Reporting System and close the case within forty-five (45) calendar days after the Critical Incident was initially reported. A case cannot be closed until the following information is provided: 7.4.4.1 A summary of any debriefings; 7.4.4.2 Whether the Individual is in custody (jail), in the hospital or in the community; 7.4.4.3 Whether the Individual is receiving services and include the types of services provided; 7.4.4.4 If the Individual cannot be located, the steps the Contractor has taken to locate the Individual using available, local resources; and 7.4.4.5 In the case of the death of an Individual, verification from official sources that includes the date, name and title of the sources.   • Convicted, Discriminatory, Antitrust Violator, and Suspended Vendor Lists In accordance with sections 287.133, 287.134, and 287.137, F.S., the Contractor is hereby informed of the provisions of sections 287.133(2)(a), 287.134(2)(a), and 287.137(2)(a), F.S. A firm or individual placed on the Suspended Vendor List pursuant to section 287.1351, F.S., the Convicted Vendor List pursuant to section 287.133, F.S., the Antitrust Violator Vendor List pursuant to section 287.137, F.S., or the Discriminatory Vendor List pursuant to section 287.134, F.S., is immediately disqualified from Contract eligibility.

  • No Felony Criminal Convictions Contractor represents that neither Contractor nor any of its employees, agents, or representatives, including any subcontractors and employees, agents, or representative of such subcontractors, have been convicted of a felony criminal offense or that if such a conviction has occurred Contractor has fully advised System Agency in writing of the facts and circumstances surrounding the convictions.

  • Abuse and Neglect of Children and Vulnerable Adults: Abuse Registry Party agrees not to employ any individual, to use any volunteer or other service provider, or to otherwise provide reimbursement to any individual who in the performance of services connected with this agreement provides care, custody, treatment, transportation, or supervision to children or to vulnerable adults if there has been a substantiation of abuse or neglect or exploitation involving that individual. Party is responsible for confirming as to each individual having such contact with children or vulnerable adults the non-existence of a substantiated allegation of abuse, neglect or exploitation by verifying that fact though (a) as to vulnerable adults, the Adult Abuse Registry maintained by the Department of Disabilities, Aging and Independent Living and (b) as to children, the Central Child Protection Registry (unless the Party holds a valid child care license or registration from the Division of Child Development, Department for Children and Families).
