Verified Reports. Developer shall completely and accurately fill out and file forms DSA 6-C or DSA 152 (or most current version applicable at the time the Work is performed), as appropriate. Refer to section 4-336 and section 4-343 of Part 1, Title 24 of the California Code of Regulations.

Verified Reports. The Contractor shall make and submit to the office from time to time, verified reports as required in Title 24 Section 4-366. As part of the Close-Out of the Project (see Article 13.16), Contractor shall be required to execute a Form 6-C as required under Title 24 Sections 4-343.
Verified Reports. The INSPECTOR shall make and submit to the Division of the State Architect verified reports pursuant to Section 3-342 of Title 24 of the California Code of Regulations. The INSPECTOR shall prepare and deliver to the Division of the State Architect detailed statements of fact regarding materials, operations, etc., when requested.
Verified Reports. Inspector shall submit verified reports at the following times that will include notification of outstanding deviations: 10.11.2.1. Work on the Project is suspended for a period of more than one (1) month. 10.11.2.2. Inspector is terminated for any reason. 10.11.2.3. DSA requests a verified report. 10.11.2.4. If District occupies any building on a Site. 10.11.2.5. When the Construction Work is complete. Each verified report shall be on Form DSA-6, or more current form, and shall clearly describe all non-compliant Construction Work including change order work that is pending DSA approval. Each verified report shall state that Inspector knows of his/her personal knowledge that the Construction Work has, in every material respect, been performed in compliance with the Construction Documents. Inspector shall declare under penalty of perjury that all information indicated on the report is true.
Verified Reports. The Inspector shall submit verified reports directly to the DSA, the Architect and the District upon any of the following events occurring: 14.2.1. Work on the project is suspended for a period of more than one (1) month. 14.2.2. The services of the Inspector are terminated for any reason prior to the completion of the project. 14.2.3. At the time of occupancy of any building, or portion of a building, involved in the project prior to the completion of the entire DSA approved scope of work. 14.2.4. When the construction is sufficiently complete, in accordance with the DSA approved construction documents, so that the District can occupy or utilize the project. 14.2.5. DSA requests a verified report. Each verified report shall be on form DSA 6-PI and forms DSA 152, or more current form, as appropriate. Reports shall be sent electronically to the DSA and kept in the Job File.
Verified Reports. (i) The INSPECTOR shall make and submit to the DSA written verified reports, on form DSA 6PI or form DSA 152 as applicable, pursuant to Section 4-342 of Title 24 of the California Code of Regulations. The INSPECTOR shall submit the appropriate verified report to the DSA upon any of the following events: (A) Work on the PROJECT is suspended for a period of more than one month; (B) The services of the INSPECTOR are terminated for any reason prior to the completion of the PROJECT; (C) At the time of occupancy of any building or portion of a building involved in the PROJECT prior to the completion of the entire DSA approved scope of work; (D) The PROJECT is substantially complete as determined by the DSA; or (E) The DSA requests a verified report. The INSPECTOR shall prepare and deliver to the DSA detailed statements of fact regarding materials, operations, etc., when requested. (ii) The INSPECTOR shall collect copies of the verified reports prepared by the Architect, Engineers, Laboratory of Record, Special Inspectors, Geotechnical Engineer, and/or the Contractor upon any of the following events: (A) Within 14 days of the completion of any of the above parties’ services or work; (B) Work on the PROJECT is suspended for a period of more than 1 month; (C) The services of any of the above parties is terminated for any reason prior to the completion of the PROJECT; or (D) The DSA requests a verified report from any of the above parties.
Verified Reports. Architect shall make the “verified reports” required by the California Education Code and Code of Regulations, according to the form and schedule required by those codes and DSA.
