Voluntary Assumption of Risk. I acknowledge and agree that I and the Child are participating voluntarily at our own risk. I acknowledge and agree that the actions or activities of other customers or the actions or inactions of Sky Zone Facility employees could cause me or the Child significant bodily injury (as described in this Agreement), and that SZ is not responsible for the actions or activities of customers using the Sky Zone Facility or the negligence of its employees in supervising the Sky Zone Facility or its usage, including actions, activities, or omissions that result in such harm. Some of the risks include, but are not limited to, the following:

Consultant acknowledges that the Services that he is required to perform under this Agreement include inherent dangers, including the risk of serious bodily injury and/or death. Voluntary Assumption of Risk. I acknowledge and agree that I and the Child are using PA Facilities and participating in PA Programs (whether or not at PA Facilities) voluntarily at our own risk. I acknowledge and agree that the actions or activities of other participants or the actions or inactions of PA respective employees and volunteers could cause me or the Child significant bodily injury (as described in this Agreement), and that the PA is not responsible for the actions or activities of participants using PA Facilities, or any other facility or location where PA Programs are held, or the negligence of its employees and volunteers in supervising PA Facilities or their usage, or any other facility or location where PA Programs are held, or their usage, including actions, activities, or omissions that result in such harm. Some of the risks include, but are not limited to, the following:
Voluntary Assumption of Risk. The Player acknowledges and agrees that: Australian Football is a vigorous body contact sport in which physical injury is likely to occur from time to time; The Player nonetheless desires to play Australian Football for the Club; The Player personally accepts the risk both physical and legal of injury arising in the course of training for and participating in a game of Australian Football; The Player will disclose to the Club any physical or mental condition or ailment that could affect the ability of the Player to carry out the Player’s obligations under this Playing Contract; and The Player will, on request by the Club, complete any reasonable questionnaire concerning the medical condition of the Player presented to the Player by the Club.

Nonetheless, I, for myself and on behalf of my Child, assume full responsibility for all risk of all bodily injury, death or property damage sustained while my Child or I participate(s) in the Activities, use the Facilities and Equipment or arising out of TransportationAuthorization.

I UNDERSTAND AND ACKNOWLEDGE THAT PARTICIPATION IN TRAMPOLINE COURT ACTIVITIES ENTAILS KNOWN AND UNANTICIPATED RISKS THAT COULD RESULT IN PHYSICAL AND/OR EMOTIONAL INJURY, PARALYSIS, DEATH OR DAMAGE TO MYSELF AND/OR OTHERS. RISKS MAY INCLUDE, BUT ARE NOT LIMITED TO, SLIPPING AND FALLING, COLLISIONS WITH FIXED OBJECTS AND/OR OTHER PEOPLE WHICH MAY RESULT IN SPRAINS, FRACTURES, BREAKS, SCRAPES, BRUISES, DISLOCATIONS AND INJURIES TO HEAD, BACK AND NECK. I ACKNOWLEDGE THAT I AM VOLUNTARILY PARTICIPATING IN THE ACTIVITIES WITH KNOWLEDGE OF THE DANGERS INVOLVED AND EXPRESSLY AND VOLUNTARILY AGREE TO ACCEPT AND ASSUME ANY AND ALL RISKS OF INJURY, DEATH, OR PROPERTY DAMAGE INVOLVED IN AND ASSOCIATED WITH PARTICIPATION IN, OR SPECTATING OF TRAMPOLINE COURT ACTIVITIES.
