WORKING DUES. The Employer agrees to deduct from the employees' wages, as set forth in Article 1 of this Appendix, working dues in the amount of forty cents per hour for each hour worked by the said employees and to remit the said working dues not later than the day of the month following the month for which the deduction is made, in the following manner: Thirty-five cents per hour for each hour worked shall be remitted to Local and the Employer shall use the Welfare Contribution Form for the remittance of the said thirty-five cents per hour and shall note thereon the employee's names, social insurance numbers and number of hours worked; and Five cents per hour for each hour worked shall be remitted to the Secretary-Treasurer of the Labourers* International Union of North America, Ontario Provincial District Council. District Council working dues' deductions shall be remitted to the Labourers' Pension Fund of Central and Eastern Canada. The Employer shall when remitting such dues submit a list of names, social insurance numbers and number of hours worked for those on whose behalf such deductions were made, on one Standard Benefits Reporting Form showing all applicable deductions and/or contributions. The above deductions shall be remitted together with Pension Fund contri- butions to the Labourers* Pension Fund of Central and Eastern Canada.
