I hereby assert that our par+cipa+on is voluntary and that I knowingly assume all such risks; and I cer+fy that I and my minor child have no medical condi+on which would cause par+cipa+on in ac+vi+es at the Loca+on to be poten+ally hazardous to our health. In addi+on, I give explicit authoriza+on for the Releasees to provide or cause to be provided such medical treatment as may be necessary or appropriate if an injury occurs while at the Loca+on; and

I, on behalf of myself, the Minor, and our respective heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue SCA, its owners, officers, directors, shareholders, employees, agents, and/or affiliates from any and all claims, liabilities, demands, causes of action, or losses, including reasonable attorneys’ fees and costs, arising out of or related to the Minor's participation in the Activities, including, but not limited to, personal injury, property damage, or wrongful death. I understand that this waiver and release of liability is intended to be as broad and inclusive as permitted by Florida law and if any portion of this waiver and release is held invalid, it is agreed that the balance shall continue in full legal force and effect to the greatest extent permitted by law.

In recognition of the risk of injury while participating in the Blackstone Public Library’s Walkathon (Event), and as consideration for the right to participate in the Event, I hereby for myself, my heirs, executors, administrators, assigns, or personal representatives knowingly and voluntarily enter into this waiver and release of liability (Agreement) and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the Event, and do hereby release and forever discharge (Event) and its affiliates, managers, members, agents, attorneys, employees, volunteers, heirs, representatives, predecessors, successors and assigns, business and its affiliates, managers, members, agents, attorneys, employees, volunteers, heirs, representatives, predecessors, successors and assigns, the event site and its agents, attorneys, employees, volunteers, heirs, representatives, predecessors, successors and assigns and all sponsors and/or beneficiaries of the Event (collectively Hosts) from any and all liability, claims, demands, damages, actions, or causes of action now existing or which hereinafter may arise as a result of my participation in the Event, whether any injury is caused by the negligence of the Hosts, the negligence of myself or third parties, the conditions of the course or any other cause.
