Action Item 2. During the duration of this Agreement, the District will provide OCR with an updated copy of the Student’s Section 504 Plan, if it is revised during the 2021 fall semester. REPORTING REQUIREMENT 2: The District will provide OCR a copy of the Student’s Section 504 Plan, if it is revised during the 2021 fall semester, within 10 business days of any revision.

  • If the Student accepts the re-evaluation offer pursuant to Action Item 2, by November 30, 2023, the District will submit to OCR documentation which evidences its completion of a Section 504/Title II evaluation of the Student and the District’s decisions regarding the Student eligibility for services pursuant to Section 504/Title II, including a copy of the Section 504 Plan or IEP prepared for the Student, and the Student’s need for any compensatory and/or remedial services.

  • By November 14, 2016 and November 14, 2017, the District will provide documentation to OCR demonstrating that training was provided consistent with Action Item 2 above, including: (a) the name(s) and credentials of the individual(s) who conducted the training; (b) a list of the individuals who attended the training and their positions; (c) the date(s) the training was conducted; (d) copies of any training materials disseminated.

  • By June 15, 2017, the District will provide documentation to OCR with a written report confirming the completion of training specified in Action Item 2.

  • By November 1, 2018, the Dallas ISD shall provide documentation to OCR evidencing that the evaluation required pursuant to Action Item #2 has been conducted.

  • Action Item 2: By December 15, 2018, the District will assess the paths of travel from the concession stand and bathrooms to the bleachers for the BGHS baseball and softball fields, to determine whether they are in compliance with the 2010 Standards, Chapter 2, Section 206, and Chapter 4, together with all associated figures and other cross-referenced provisions.
