Covered Personal Information definition

Covered Personal Information means the following information the Company collects, uses or discloses from or about an individual: (i) first and last name; (ii) home or other physical address, including street name and city or town; (iii) email address or other online contact information, such as a user identifier or screen name; (iv) persistent identifier, such as IP address or machine I.D.; (v) telephone number, including home telephone number and mobile telephone number; (vi) physical location; or (vii) any other information from or about an individual consumer that alone or in combination with other information could be used to identify an individual or otherwise facilitate decisions regarding individuals.
Covered Personal Information means information the Company or any of its Subsidiaries collects, uses or discloses from or about an individual, including, but not limited to, an individual’s: (i) first and last name; (ii) home or other physical address, including street name and city or town; (iii) email address or other online contact information, such as a user identifier or screen name; (iv) persistent identifier, such as IP address or machine I.D.; (v) telephone number, including home telephone number and mobile telephone number; (vi) physical location; or (vii) any other information from or about an individual consumer that alone or in combination with other information could be used to identify an individual or otherwise facilitate decisions regarding individuals.
Covered Personal Information means the following information the Company or any of its subsidiaries collects, uses or discloses from or about an individual: (i) first and last name; (ii) home or other physical address, including street name and city or town; (iii) email address or other online contact information, such as a user identifier or screen name; (iv) persistent identifier, such as IP address or machine I.D.; (v) telephone number, including home telephone number and mobile telephone number; (vi) physical location; (vii) Protected Health Information (as such term is defined in HIPAA); or (viii) any other information from or about an individual consumer that alone or in combination with other information could be used to identify an individual or otherwise facilitate decisions regarding individuals.

More Definitions of Covered Personal Information

Covered Personal Information means any personal information provided by or on behalf of Third Party Provider, a CCPA Affiliate or any consumer, or otherwise made available to Service Provider, pursuant to the Service Provider Agreement(s) or in connection with the Services.

Covered Personal Information means the following information the Company collects, uses or discloses from or about an individual: (A) a natural person’s name, street address, telephone number, e-mail address, photograph, social security number or tax identification number, driver’s license number or state identification number, passport number, credit or debit card number, insurance policy number, financial information, or customer or account number, health information, consumer report information, device identifiers, transaction identifier, IP addresses, physiological and behavioral biometric identifiers or any other piece of information that alone or in combination with other information directly or indirectly collected, held or otherwise managed by or for the Company allows the identification of or contact with a natural person or a particular computing system or device, or with respect to which there is a reasonable basis to believe it can be used to identify a natural person (and which, for greater certainty, includes all such information with respect to employees), and (B) any information that is associated, directly or indirectly (by, for example, records linked via unique keys), to any of the foregoing.

Covered Personal Information means the following information the Transferred Subsidiary and KI collect, use or disclose or a person collects, uses or discloses on behalf of the Transferred Subsidiary and KI from or about an individual: (i) first and last name; (ii) home or other physical address, including street name and city or town; (iii) email address or other online contact information, such as a user identifier or screen name; (iv) persistent identifier, such as internet protocol address or machine, device or advertising identification, or equivalent; (v) telephone number, including home telephone number and mobile telephone number; (vi) account number or identifier of any type; (vii) “Protected Health Information” (as such term is defined in HIPAA); (viii) Nonpublic Personal Information (as such term is defined in GLBA) or (ix) any other information from or about an individual consumer that could reasonably be used to identify an individual.
Covered Personal Information means any personal information or personal data provided by Employer to Prime Retirement Solutions, LLC, collected by Prime Retirement Solutions, LLC on behalf of Employer, processed by Prime Retirement Solutions, LLC on behalf of Employer, or otherwise made available to Prime Retirement Solutions, LLC pursuant to an Engagement Letter or Client Service Agreement.
Covered Personal Information means any Personal Information provided by or on behalf of Customer to GHD, collected by GHD on behalf of Customer, or otherwise made available to GHD pursuant to the Agreements. Covered Personal Information does not include “aggregate consumer information” or “deidentified” data as defined in the State Privacy Laws, or other information which is not “personal information” as defined under Applicable Privacy Laws.
