Eligibility for Leave. Employees who have completed their 90-day probationary period may request a leave. However, to be eligible for FMLA leave an Employee must have worked for the Company (at the start date of the requested leave): (a) For at least twelve (12) months, and (b) For at least 1,250 hours during the twelve (12) month period prior to the leave request.
