Eligibility for Leave. Employees who have completed their 90-day introductory periods may request a leave. However, to be eligible for FMLA leave an employee must have worked for the Company (as the start date of the requested leave): 1. for at least twelve (12) months, AND 2. for at least 1,250 hours during the twelve (12) month period prior to the leave request.

For items that are mailed or shipped by the provider, the date of service is the date on which the order was received, the date on which the item was fabricated, or the date on which the item was mailed or shipped.

  • Protected Employees means employees of the Company who were employed by the Company at any time within six (6) months prior to the Determination Date.

  • Managing Employee means a general manager, business manager, administrator, director, or other individual who exercises operational or managerial control over, or who directly or indirectly conducts the day-to-day operation of an institution, organization, agency or business entity;
