Examples of High Ongoing Support in a sentence
If a Participant receiving Moderate Ongoing Support or High Ongoing Support is Suspended for more than 12 months, the Provider must perform a Provider Exit of the Participant.
If at any time the Provider considers, in accordance with any Guidelines, that a Disability Employment Services – Disability Management Service Participant requires Moderate Ongoing Support or High Ongoing Support, the Provider must arrange a Change of Circumstances Reassessment in accordance with clause 129 [Change of Circumstances Reassessment during Ongoing Support].
If at any time an OSA recommends that a Disability Employment Services – Disability Management Service Participant receive Moderate Ongoing Support or High Ongoing Support, then the OSA shall be considered a Change of Circumstances Reassessment, and the provisions of clause 129 [Change of Circumstances Reassessment during Ongoing Support] apply.
The OSA recommends Flexible, Moderate or High Ongoing Support for a DES - ESS Participant: Proceed to Step 5.
Where the Assessor recommends Moderate or High Ongoing Support for a Disability Management Service (DMS) Participant, the Provider must move the Participant to Employment Support Service (ESS) and deliver the recommended Ongoing Support level at the next regular contact.
Moderate or High Ongoing Support- Participant must transfer to an DES-ESS Provider if they received Work Assist from a Provider that only delivers the DES-DMS; Participant can remain with their current Provider if they received Work Assist Services under the DES-ESS, or if their DES‑DMS Provider also provides DES-ESS.
The Provider will transfer a DES – DMS Participant to a new DES Provider if they are not contracted to provide the Program Services recommended in the OSA in their ESA (i.e. Moderate or High Ongoing Support).
The recommendation will be for one of the following: Post-Placement Support (no Ongoing Support required, and the Participant is working toward a 52-week Outcome); Exit (no Ongoing Support required, and the Participant has achieved a 52-week outcome); Flexible Ongoing Support; Moderate Ongoing Support – (for Employment Support Service Participants only); or High Ongoing Support – (for Employment Support Service Participants only).
High Ongoing Support (Employment Support Service Participants only) The Participant’s requirements for support while in Post Placement Support, or during the period since their previous OSA was conducted, whichever is later, were: Regular and ongoing, Significant support either in hours or intensity, and At least twelve contacts in the last three months, or More than 42 hours of Support in the last six months.
