Host Plan definition

Host Plan means the on-site Blue Cross/ Blue Shield Plan, which services the geographic area outside the Service Area where the covered services are provided.
Host Plan means, with respect to each Temporary Participant, the Plan maintained by a Fund to which employer contributions are required on account of a Participant's employment subsequent to this commencement of participation in the Primary Plan.

  • The methods used by a Host Plan to determine a negotiated price will vary among Host Plans based on the terms of each Host Plan’s provider contracts.

  • Statutes in a small number of states may require a Host Plan to either (1) use a basis for calculating the Member’s liability for covered health care services that does not reflect the entire savings realized, or expected to be realized, on a particular claim or (2) add a surcharge.

  • Under the BlueCard Program, recoveries from a Host Plan or from participating providers of a Host Plan can arise in several ways.

  • In some cases, the Host Plan will engage a third party to assist in identification or collection of recovery amounts.

  • If Blue Cross and Blue Shield enters into a Negotiated National Account Arrangement with a Host Plan to provide Value-Based Programs to your Members, Blue Cross and Blue Shield will follow the same procedures for Value-Based Programs administration as noted in the BlueCard Program section.

  • The BlueCard Program enables you to obtain covered services outside of California, as de- fined, from a health care provider participating with a Host Plan, where available.

  • The BlueCard Program requires that the price submitted by a Host Plan to Blue Cross and Blue Shield is a final price irrespective of any future adjustments based on the use of estimated or average pricing.

Related to Host Plan

  • layout plan means a plan of the project depicting the division or proposed division of land into plots, roads, open spaces, amenities, etc. and other details as may be necessary;

  • Service Plan means the Operating Plan and Budget appended as Schedules A and D2a of Schedule D;

  • flight plan means specified information provided to air traffic services units, relative to an intended flight or portion of a flight of an aircraft;

  • Project Implementation Plan means the detail plan submitted by the Developer with regard to development of Project Facilities and its operation and management thereof in accordance with this Agreement and to be appended as Schedule 9 to this Agreement.

  • Project Management Plan means the management plan that (i) sets out a high level workplan to describe the manner in which the Design-Builder will manage the Project, including to address related matters such as traffic management and communications, and (ii) is prepared by or for the Design-Builder and submitted to the Owner;

  • Flexi Plan means any individual indemnity hospital insurance plan under the VHIS framework with enhancement(s) to any or all of the protections or terms and benefits that the Standard Plan provides to the Policy Holder and the Insured Person, subject to certification by the Government. Such plan shall not contain terms and benefits which are less favourable than those in the Standard Plan, save for the exception as may be approved by the Government from time to time.

  • Procurement Plan means the Recipient’s procurement plan for the Project, dated April 2, 2010, and referred to in paragraph 1.16 of the Procurement Guidelines and paragraph 1.24 of the Consultant Guidelines, as the same shall be updated from time to time in accordance with the provisions of said paragraphs.

  • BT Network means the communications network owned or leased by BT and used to provide the Service.

  • Master Plan means a subcontracting plan that contains all the required elements of an individual contract plan, except goals, and may be incorporated into individual contract plans, provided the master plan has been approved.

  • Services plan means a written statement that describes the special education and related services the school district will provide to a parentally-placed child with a disability enrolled in a private school located in the district, including the location of the services and any transportation necessary, consistent with 34 CFR §§ 300.132, 300.137-139.

  • Transition Plan means a transition plan, acceptable to the LHIN that indicates how the needs of the HSP’s clients will be met following the termination of this Agreement and how the transition of the clients to new service providers will be effected in a timely manner; and

  • Implementation Plan means the schedule included in the Statement of Work setting forth the sequence of events for the performance of Services under the Statement of Work, including the Milestones and Milestone Dates.

  • Management Plan means a plan to manage the activities and protect the special value or values in an Antarctic Specially Protected Area or an Antarctic Specially Managed Area.

  • Supply Plan has the meaning set forth in the CAISO Tariff.

  • Business Continuity Plan means any plan prepared pursuant to clause H5.6, as may be amended from time to time.

  • Improvement Plan means the plan required by the Authority from the Supplier which shall detail how the Supplier will improve the provision of the Goods and/or Services pursuant to Clause 29.1.1 (Authority Remedies);

  • Recovery Plan means a recovery plan drawn up and maintained by an institution in accordance with Article 5;

  • Project Implementation Manual or “PIM” means the manual setting out the measures required for the implementation of the Project, as the same may be amended from time to time, subject to prior approval of the Association;

  • Area network means a type of electric distribution system served by multiple transformers interconnected in an electrical network circuit, which is generally used in large metropolitan areas that are densely populated, in order to provide highly reliable service. Area network has the same meaning as the term "distribution secondary grid network" found in institute of electrical and electronics engineers (IEEE) standard 1547 sub clause 4.1.4.

  • Payment Plan means the schedule of payment prescribed in Schedule C;

  • Development Plans has the meaning set forth in Section 3.2.

  • Parent Plan has the meaning set forth in Section 7.8(b).

  • Training Plan means the plan that outlines what training and assessment will be conducted off-the-job and what will be conducted on-the-job and how the Registered Training Organisation will assist in ensuring the integrity of both aspects of the training and assessment process.

  • Program Plan means the tobacco settlement program plan dated February 14, 2001, including exhibits to the program plan, submitted by the authority to the legislative council and the executive council, to provide the state with a secure and stable source of funding for the purposes designated by section 12E.3A and other provisions of this chapter.

  • Project Plan means the document to be developed by the Contractor and approved by Webel Technology Ltd., based on the requirements of the Contract and the Preliminary Project Plan included in the Contractor’s bid. For the sake of clarity, the Agreed and Finalized Project Plan” refers to the version of the Project Plan submitted by the contractor after receiving the letter of Award and the same approved by Webel Technology Ltd. The project plan may be changed/ modified during the course of the project. Should the Project Plan conflict with the provisions of the Contract in any way, the relevant provisions of the Contract, including any amendments, shall prevail.

  • Educational program means a program for educating and preparing physician assistants which is approved by the board.
