Proof of Disability definition

Proof of Disability. The County shall have the right to re- quire the submission of adequate medical proof of the employee's disability due to accident or illness. Should there be an extended period of disability, the County shall have the right to require periodic medical proof of the employee's disability. Injury or Illness on Job: If any employee appears to be in- jured or ill while on the job, or there is reason to believe that an employee needs medical attention, his supervisor shall have the right to require the employee to furnish a statement from a licensed physician before returning to work that the employee is capable of performing the work required by his job. The County shall send such employee to the doctor at its expense on working time.
Proof of Disability. ’ means an affidavit issued by a registered medical practitioner confirming that the person is disabled.
Proof of Disability means a statement of disability signed by a physician or a verification of federally issued disability benefits.

Examples of Proof of Disability in a sentence

  • Proof of disability must be submitted within six (6) months following the Qualifying Period.

  • Proof of disability will be required before such payments are made.

  • Proof of disability satisfactory to the insurer, must be provided at the request of the insurer.

  • Proof of disability signed by a practicing physician shall be submitted with the leave request.

  • Proof of disability will be required before such advances are made.

  • Proof of disability, as for sick leave regulations, will be required before advances are made.

  • Proof of disability, satisfactory to the insurer, must be provided at the request of the insurer.

  • Proof of disability must be filed within three months after total disability has lasted nine months.

  • However, a participant can request that benefits be paid for late claims if a participant can show that: • It was not reasonably possible to give written proof of disability during the one (1) year period and, • Proof of disability satisfactory to Insurance Provider was given to Insurance Provider as soon as was reasonably possible.

  • Proof of disability must be submitted within six months following the Qualifying Period.
