Request for Service definition
Request for Service or “RFS” means the document used by the County Representative to obtain services under this Contract. This RFS must include a description of required services and schedule. It is to be emailed or faxed to the Contractor who in turn will provide a detailed costing for services. The RFS must be reviewed and approved by the County Representative before the work is to proceed. A sample of the RFS Form is attached as Schedule 2 and detailed Contractor Engagement Process is attached as Schedule 3.
Request for Service means a Co-Location Request, Request for Co-Location Space, Request for Connection Service, Request for Building Lead-in Duct and Lead-in Manhole, Request for Distribution Frame Mounting, Requesting for Internal Wiring, Request for Interconnect Capacity without Forecast, Request for Line Sharing, Request for Local Loop or Sub Loop, or Request for Roof Access (as the case may be);
Request for Service means the point in time when services are sought or applied for through a telephone call, walk-in, or written request for services from an enrollee or the Authorized Representative. For purposes of this Agreement, an EPSDT referral is only a Request for Service when the enrollee or the person authorized to consent to treatment for that enrollee has confirmed that the enrollee is requesting service.

This Agreement shall come into force on the Commencement Date and, unless terminated as provided for in clause 10 or 11, shall continue in force until the final Accepted Request for Services has expired or been terminated (the “Term”) provided that the Customer may at any time terminate any Accepted Request for Service, for convenience and without cause, by giving INX-ZA notice as specified in clause 4.1.
