Service delivery site definition

Service delivery site means those sites owned or occupied by the Authority to which the Contractor must deliver Services, which are identified in the Output Based Specification, and further details of some of which are set out in Part A (Existing Locations) of Schedule 4 (Site Issues);
Service delivery site means a medical office, facility or other location where the PROVIDER-MANAGER provides Primary Care Services to HMO Members assigned to the PROVIDER-MANAGER, as specified in Attachment D.
