Accident and Illness While on Leave of Absence Interruption while on leave caused by serious illness or accident, evidence of which is satisfactory to the Superintendent/President or the Governing Board, shall not be held against an employee with regard to the fulfillment of the conditions under which the leave is granted, provided that the Superintendent/President has been promptly notified of such accident or illness. This notification shall be made by registered letter, mailed within thirty (30) days of the time of the accident or the onset of the illness.

  • Industrial Accident and Illness Leave shall be granted for illness or injury incurred within the course and scope of an employee's assigned duties. The employee who has sustained a job-related injury shall report the injury on an Office approved accident form to the immediate supervisor within twenty-four (24) hours. An employee shall report any illness, in writing, to the immediate supervisor within twenty-four (24) hours of knowledge that the illness is an alleged industrial illness.   • Accident and Incident Investigations An accident, injury, critical injury, or hazardous situation will be investigated in accordance with the CJHSC’s investigation protocol.
