Accident and Incident Investigations. An accident, injury, critical injury, or hazardous situation will be investigated in accordance with the CJHSC’s investigation protocol.

Lockout Program Employees who may be at risk because they are required to set up or to repair or maintain machinery, equipment or systems where lockout is required, shall receive lockout training. The Employer shall provide employees with sufficient numbers of personal locks to ensure that all equipment is locked out before being repaired, maintained or set up. No supervisor or employee shall remove another employee=s lock without prior notification except as provided in the Lockout Tagout Protocol and Procedure dated June 1, 2006. All new employees shall be trained within six (6) months of the start of their employment.
