Additions to Personnel Record. Staff members shall be copied on any documents relating to their performance which are placed in their personnel record including those maintained by departmental offices and Human Resources. Letters of expectation will be placed only in a staff member’s departmental office personnel record.   • Access to Personnel File Each employee shall have reasonable access to his/her personnel file for the purpose of reviewing any evaluations or formal disciplinary notations contained therein, in the presence of the Director of Personnel or designate. An employee has the right to request copies of any evaluations in this file.

  • Personnel Record Agencies should reimburse the employee using an employee Expense Reimbursement Document. Then the agency’s Payroll staff should do a one-time override using DBA code 5700 for qualified (non-taxable) expenses and/or DBA code 5800 for non- qualified (taxable) expenses.
