ANNUAL MASTER CONTRACT SALES REPORT. Contractor shall provide to Enterprise Services a detailed annual Master Contract sales report. Such report shall include, at a minimum: Product description, part number or other Product identifier, per unit quantities sold, and Master Contract price. This report must be provided in an electronic format that can be read by MS Excel.

ANNUAL MASTER CONTRACT SALES REPORT. Contractor shall provide to Enterprise Services a detailed annual Master Contract sales report. Such report shall include, at a minimum: the Goods/Services sold (including, as applicable, item number or other identifier), per unit quantities sold, items and volumes purchased by Purchaser, shipment/delivery locations by Purchaser, and Master Contract price. Such report is due within thirty (30) calendar days of the annual anniversary of the effective date of this Master Contract.

Insurance Endorsements: Timely provide to Enterprise Services at the designated address, without exception, annual insurance endorsements for the insurance coverages required by this Master Contract. Vendor Management Fee: Timely remit to Enterprise Service, with no less than a 75% on time rate over the contract term, the applicable Vendor Management Fee (VMF). If not, why? o Did Contractor complete the project below the target budget? If not, why? o Did Contractor recycle old cabling that was removed? If so, how much? Additional reports may include requests from the State Efficiency and Environment Performance Office (SEEP), which may vary year to year. Other required reports will be designed and requested by the Contract Administrator. Contractor will provide a response within one week and provide the requested data within two weeks.

Such Affidavit of Amounts Paid either shall state, if applicable, that Contractor still maintains its MWBE certification or state that its subcontractor(s) still maintain(s) its/their MWBE certification(s) and specify the amounts paid to each certified MWBE subcontractor under this Master Contract. Contractor shall maintain records supporting the Affidavit of Amounts Paid in accordance with this Master Contract’s records retention requirements.
ANNUAL MASTER CONTRACT SALES REPORT. Annually, within thirty (3) days of the anniversary of the effective date of this Master Contract, Contractor shall provide to Enterprise Services a detailed annual Master Contract sales report. Such report shall include, at a minimum: Total dollar amount collected for Purchasers, by Purchaser, total amount of debt, total debt collection service fees collected, by Purchaser, and applicable debt collection service fees.
