Common use of Annual Physicals Clause in Contracts

Annual Physicals. Effective October 1, 2019, employees are eligible for reimbursement of up to $250.00 dollars of the coinsurance cost of an annual physical. The following criteria must be met in order to be eligible for reimbursement: •Only one physical will be covered by reimbursement within a fiscal year. •Each physical must be six months or greater from the prior year’s physical. •At a minimum, the physical must meet the same standards as a pre-employment physical required by the City of Porterville Fire Department. •The physical must be conducted by an in-network physician. •Any and all additional expenses related to the annual physical will not be covered by the employer (i.e. travel, lodging, etc.).
