APPLICATION FOR USE OF LEAVE. Application for use of such leave, except in emergency cases, shall be made to the Department Head or his/her designee not less than three (3) work days in advance of each anticipated absence for personal business. The granting of such leave shall be subject to the reasonable operating needs of the department, and any denial of the use of personal leave shall be given to the employee at the time of his/her request. It is understood that personal leave may be taken in hours.

APPLICATION FOR USE OF LEAVE. 1. An employee requesting sick leave for a scheduled medical appointment shall complete the appropriate leave usage form and submit the form to the department head as soon as possible prior to the date of the scheduled medical appointment.

  • Abuse of Leave If it is found that a leave is not actually being used for the purpose for which it was granted, the Employer may cancel the leave and direct the employee to report for work by giving written notice to the employee.

  • Use of Leave An employee does not need to use this leave entitlement in one block. Leave can be taken intermittently or on a reduced leave schedule when medically necessary. Employees must make reasonable efforts to schedule leave for planned medical treatment so as not to unduly disrupt the employer’s operations. Leave due to qualifying exigencies may also be taken on an intermittent basis.

  • Application for Leave Any request for a leave of absence shall be submitted in writing by the employee to the Employer or designee. The request shall state the reason the leave of absence is being requested and the length of time off the employee desires. Authorization for a leave of absence shall be furnished to the employee by the Employer or designee, and it shall be in writing.

  • Application for Use a. The Employer agrees to accept properly executed leave applications within six (6) months of the first day of the period of leave being requested.

  • Application for Sabbatical Leave 16.K.2.a. The application for sabbatical leave shall include a statement of the relationship between the proposed sabbatical activity(ies) and the applicant’s current or prospective service to the College and of the benefit(s) that the District shall accrue because of the leave.

  • Notice of Leave In any case in which the necessity for FMLA leave is based upon an expected birth or placement for adoption, or planned medical treatment for a serious health condition of the employee, family member, or member of the employee’s household, the employee will provide the Superintendent with at least sixty (60) calendar days advance written notice of the date the leave is to begin. When the need for FMLA leave is due to unforeseen circumstances and advance notice is not practicable, the employee will provide notice of the need for such leave as soon as practicable.
