Application for Leave of Absence. 25.01 All leaves of absence without pay and any extension thereof must be applied for in writing to the Mayor or his designee, on forms supplied by the Employer, at least ten (10) working days prior to the proposed commencement of the leave except in serious or unusual circumstances, as determined by the Employer. Notification of the approval or denial of their requested leave shall be given to the employee in writing within five (5) working days after the submission of the request. Any denial of a requested leave shall include the reason for the denial.

An employee who requests an unpaid leave of absence shall make written application for such leave on a form provided by the District and shall submit such form to the employee’s Xxxx, Director or Manager as far in advance as possible. The application for leave shall state the leave category requested, the reason(s) necessitating the employee’s absence, and the period of time for which the leave is requested. The disposition of such request will be provided to the employee in writing by an appropriate official of the District. If leave is denied, the employee may submit a request to the appropriate Vice President for disposition.

Any extensions of leave of absence shall be handled in the same manner as the request for regular leave.

For (a) bereavement; (b) personal necessity (1), (3), (4) and (5); or (c) disability leave, the employee shall inform his/her Xxxx, Director or Manager of the absence as soon as possible and the employee will make written application for the leave as soon as practicable. The application for leave shall state the leave category requested, the reason(s) necessitating the employee’s absence, and the estimated duration of the absence. Approval of the leave shall be in writing signed by the Xxxx, Director or Manager of the employee’s division. If denied, the employee will be provided with a statement in writing giving the reason(s) for such denial.

The immediate supervisor shall forward the leave request through their President or Vice Chancellor to the Payroll and Benefits Manager. If the leave is required for planned medical treatment or supervision, the faculty member shall make reasonable efforts to schedule the supervision or treatment to avoid disruption to the District. The date of return from any leave under this section shall be established in advance by mutual agreement of the Board and the teacher.

Exceptions shall be made under extenuating circumstances. The Board reserves the right to limit to a reasonable number the employees on leave at one time. The request for the leave of absence must be in writing setting forth the reasons for the leave of absence and the anticipated length of time; and if requested, verification shall be submitted. Any extensions for leaves of absence shall be handled the same as the request for the regular leave. In instances of unpaid absence caused by non-health related conditions (e.g., vacation trip and/or for personal reasons not covered in Section A of this article), the employee may take a total of five (5) unpaid days off before becoming responsible for the employer’s share of the per diem cost of health insurance. On the sixth and succeeding such days the employee shall be responsible for the Board of Education’s portion of the per diem cost of the health insurance premium while on unpaid leave of absence.
