Assistance to Patients. TAF shall interact with patients in a manner designed to ensure that the provision of any financial assistance does not improperly influence any patient’s selection of a particular provider, practitioner, supplier, product, or test. At a minimum, TAF shall implement the following policies and practices:‌ a. TAF shall assist all eligible, financially needy patients on a first-come, first-served basis to the extent funding is available. Patients shall not be eligible for assistance unless they meet TAF’s financial need eligibility criteria and the patient has been diagnosed with a disease covered by the fund. TAF shall provide assistance on a fund-by-fund basis that is based on a reasonable, verifiable, and uniform measure of financial need that is applied to all patients within the fund in a consistent manner. b. TAF shall establish a uniform process for screening all applicants for compliance with a fund’s designated financial eligibility and medical criteria (Screening Process) prior to enrolling applicants in a fund or within a reasonable amount of time thereafter. TAF shall not require that an applicant demonstrate an inability to receive financial assistance from other sources before applying for assistance from TAF’s fund. c. TAF’s determination of a patient’s qualification for assistance shall be based on his or her financial need and a determination that the patient has a disease that is covered by the fund. In assessing a patient’s financial need, TAF shall not consider the identify of any of his or her health care providers, practitioners, suppliers, products or tests; the identity of any referring party; or the identity of any Donor that may have contributed for the support of the patient’s specified disease or the amount of the donation. d. TAF shall award assistance in a manner that xxxxxx any link between Donors and patients. TAF shall not provide patients with any information regarding Donors or the identity of Donors to the fund from which patient receives assistance. TAF shall communicate a decision to provide a grant or other financial assistance to a patient only to the patient or a personal representative (who is acting on behalf of the patient in seeking financial assistance from XXX) and who is not acting on behalf of any Donor to the fund. TAF shall not communicate such decision to a third party such as a hub.‌ e. TAF shall make no referrals or recommendations regarding specific providers, practitioners, suppliers, tests, or products....

  • Data Necessary to Perform Services The Trust or its agent shall furnish to USBFS the data necessary to perform the services described herein at such times and in such form as mutually agreed upon.

  • DEVELOPMENT OR ASSISTANCE IN DEVELOPMENT OF SPECIFICATIONS REQUIREMENTS/ STATEMENTS OF WORK

  • Assistance and Cooperation (a) Each of the Companies shall provide (and shall cause its Affiliates to provide) the other Companies and their respective agents, including accounting firms and legal counsel, with such cooperation or information as they may reasonably request in connection with (i) preparing and filing Tax Returns, (ii) determining the liability for and amount of any Taxes due (including estimated Taxes) or the right to and amount of any refund of Taxes, (iii) examinations of Tax Returns, and (iv) any administrative or judicial proceeding in respect of Taxes assessed or proposed to be assessed. Such cooperation shall include making available, upon reasonable notice, all information and documents in their possession relating to the other Companies and their respective Affiliates as provided in Section 9. Each of the Companies shall also make available to the other Companies, as reasonably requested and available, personnel (including employees and agents of the Companies or their respective Affiliates) responsible for preparing, maintaining, and interpreting information and documents relevant to Taxes. (b) Any information or documents provided under this Section 8 or Section 9 shall be kept confidential by the Company or Companies receiving the information or documents, except as may otherwise be necessary in connection with the filing of Tax Returns or in connection with any administrative or judicial proceedings relating to Taxes. Notwithstanding any other provision of this Agreement or any other agreement, in no event shall any of the Companies or any of their respective Affiliates be required to provide the other Companies or any of their respective Affiliates or any other Person access to or copies of any information if such action could reasonably be expected to result in the waiver of any Privilege. In addition, in the event that any of the Companies determine that the provision of any information to the other Companies or their respective Affiliates could be commercially detrimental, violate any law or agreement or waive any Privilege, the Parties shall use reasonable best efforts to permit compliance with their obligations under this Section 8 or Section 9 in a manner that avoids any such harm or consequence.

  • Access to Personnel File Each employee shall have reasonable access to his/her personnel file for the purpose of reviewing any evaluations or formal disciplinary notations contained therein, in the presence of the Director of Personnel or designate. An employee has the right to request copies of any evaluations in this file.

  • Authorization to Perform Services The Consultant is not authorized to perform any services or incur any costs whatsoever under the terms of this Agreement until receipt of a written Notice to Proceed from the City.

  • Outside Professional Activities 27.01 Outside professional activities conducted with professional and academic responsibility can enhance the reputation of the University and the abilities of its academic staff and librarians. This article applies only to outside professional activities that involve the application of special skills and knowledge within the member’s particular professional expertise. While a member has a primary obligation to fulfil their University responsibilities, they have the right to engage in outside professional activities subject to the following conditions: 27.01.01 A member shall notify the Xxxx/University Librarian in writing of the type, time commitment, and location of any such proposed professional activity. If the proposed activity involves the teaching at another educational institution of a course related to the faculty member's professional expertise, the faculty member shall obtain the prior written permission of the Xxxx/University Librarian, and such permission shall not be unreasonably withheld; 27.01.02 Such activities shall not require the commitment of a block of time on a regular basis which might interfere with the faculty member's normal timetable for teaching activities or a librarian's performance of normal professional responsibilities. For faculty, such activities shall not require the member to devote more than an average of one working day per five-day week on an annual basis. For librarian members, such activities shall not require the member to devote more than 20 working days per calendar year. 27.01.03 Such activities shall not hinder the fulfilment of the member's obligations to the University; 27.01.04 The name of the University and University letterhead shall not be used in such activities unless agreed to in advance by the President, although nothing shall prevent a member from stating the nature and place of employment, rank, and title in connection with such professional activities, provided that they shall not purport to represent the University or to speak for it, or to have its approval, unless such approval has been given in advance in writing by the Xxxx/University Librarian; 27.01.05 Use of University facilities, equipment, supplies, services or other personnel shall require prior written authorization from the member's Xxxx/University Librarian or from the appropriate administrative officer. Financial arrangements for such use shall be made in advance. University activities shall have priority in the use of such facilities, equipment, supplies, services and personnel; 27.01.06 A member engaged in outside professional activities shall hold the University harmless against any loss or damage that the University may suffer from such activities; and, 27.01.07 In preparing their Annual Report, each member shall include a statement of their outside professional activities in the previous year. 27.02 If the Xxxx/University Librarian has cause for concern, the Xxxx/University Librarian has the right to review a member's current or proposed outside professional activities and may require the member to provide information concerning the time devoted to current and past outside professional activities. The Xxxx/University Librarian shall inform the member in writing of the results of the review and, if necessary, of any corrective action which might be required of the member.

  • Routing for Operator Services and Directory Assistance Traffic For a Verizon Telecommunications Service dial tone line purchased by CBB for resale pursuant to the Resale Attachment, upon request by CBB, Verizon will establish an arrangement that will permit CBB to route the CBB Customer’s calls for operator and directory assistance services to a provider of operator and directory assistance services selected by CBB. Verizon will provide this routing arrangement in accordance with, but only to the extent required by, Applicable Law. Verizon will provide this routing arrangement pursuant to an appropriate written request submitted by CBB and a mutually agreed-upon schedule. This routing arrangement will be implemented at CBB's expense, with charges determined on an individual case basis. In addition to charges for initially establishing the routing arrangement, CBB will be responsible for ongoing monthly and/or usage charges for the routing arrangement. CBB shall arrange, at its own expense, the trunking and other facilities required to transport traffic to CBB’s selected provider of operator and directory assistance services.
