Certificate of Illness. Evidence in the form of a physician's certificate or certificate of illness shall be furnished as proof of adequacy of the reason for the employee's absence during the time when sick leave was requested. Certificates may be required by the Fire Chief or designee when there is an absence in excess of three (3) days; or whenever there is reason to believe that the sick leave is being abused.
