Certification of Health Sample Clauses

Certification of Health. Crewmember certifies that he or she is in good health and has no physical disabilities which may affect his or her ability to perform required duties during the term of this Agreement. Crewmember certifies in particular that he or she has no pre-existing back or neck condition and that Crewmember has not or is not now receiving medical treatment except as noted on the attached medical history report.

Certification of Health. Participant certifies that Participant is physically fit, has sufficiently prepared or trained for participation in each and every of the Activities related to the Event, where applicable, and has not otherwise been advised to not participate in such Activities by a qualified medical professional. Participant certifies that there are no health-related reasons or problems which preclude Participant’s participation in any of the Activities related to the Event.

Certification of Health. The Resident Physician hereby certifies that he/she is of sound mind and in good health and agrees to undergo a standard employment physical examination in the Hospital's Employee Health Service. In the event of the discovery of or development of a health condition of such nature which may cause a hazard to the Resident Physician or to any patient of the Hospital, or in the event that such a condition significantly impairs the Resident Physician's ability to function in his/her assigned duties, this contract may be terminated.
