Change Order Approval Process. After the Change Order is executed, one (1) copy of the Change Order, including the supporting documentation, is to be sent to Ohio EPA for final review. The HSTS Change Order form must have original signatures. Health Departments should submit change orders electronically to the DEFA Engineer who reviewed and approved their project. After the Change Order is accepted and eligible costs determined, Ohio EPA will return a signed copy of the HSTS Change Order form.

Change Order Approval Process. After the change order is executed, a minimum of three copies are to be sent to Ohio EPA ‐ DEFA for final review. All three copies must have original signatures. Only one copy of the supporting documentation for the change is to be submitted. After the Change Order is accepted and WPCLF eligible costs determined, Ohio EPA will issue a letter informing the LHD and authorizing OWDA to disburse funds from Project Contingency for the work. Ohio EPA ‐ DEFA will retain one copy of the Change Order plus the supporting documentation and send the remaining two copies to the Ohio Water Development Authority (OWDA) for processing. OWDA will retain one copy of the Change Order and send the remaining copies, signed by both Ohio EPA ‐ DEFA and OWDA, back to the LHD.

Owners have the option to submit hard copies of the project Change Orders via mail to Ohio EPA or to send PDF Change Order forms and supporting documentation electronically. With either hard copy or electronic submittals, the WPCLF Change Orders should be submitted to DEFA. The dedicated e-mail address for the electronic submittal of WPCLF Change Orders is: XXXXXXXXXX@xxx.xxxx.xxx. After the Change Order is accepted and eligible costs determined, Ohio EPA will issue a letter informing the Owner and authorizing OWDA to disburse funds from Project Contingency for the work. The OEPA letter will be sent electronically. OWDA will return a copy of the WPCLF/WSRLA Change Order form which will be signed by all parties including Ohio EPA and OWDA. Please notify Ohio EPA if the community prefers a hard copy of change order approval documentation and then Ohio EPA and OWDA will send hard copies of approval documentation through the mail.
Change Order Approval Process. In the event that the Contractor requests a change order claim that would impact the construction cost for the Cost Share Project, the engineer of record shall review and make a recommendation for approval or denial of the claim. The change order claim and the engineer’s recommendation shall both be sent to both the Developer and to the County Project Manager. The County Project Manager shall review the change order claim and the engineer’s recommendation within three (3) business days of receiving all pertinent information. For change order claims which are less than 20% of the Contract Value for the Cost Share Project, the County Project Manager shall be authorized to approve or reject proportional reimbursement for the claim on behalf of the County. Claims equal to or greater than 20% of the Contract Value for the Cost Share Project shall be reviewed and approved or rejected for proportional reimbursement by the County Administrator, or designee, who shall make every effort to do so in a timely manner. The approval or rejection of any change order claim by either the County Project Manager or the County Administrator shall be in writing, and, in the case of a rejection, shall set forth the reason(s) for rejection.

  • Change Orders and Contract Amendments 33.1 The Procuring Entity may at any time order the Supplier through notice in accordance GCC Clause 8, to make changes within the general scope of the Contract in any one or more of the following: a) drawings, designs, or specifications, where Goods to be furnished under the Contract are to be specifically manufactured for the Procuring Entity; b) the method of shipment or packing; c) the place of delivery; and d) the Related Services to be provided by the Supplier. 33.2 If any such change causes an increase or decrease in the cost of, or the time required for, the Supplier's performance of any provisions under the Contract, an equitable adjustment shall be made in the Contract Price or in the Delivery/Completion Schedule, or both, and the Contract shall accordingly be amended. Any claims by the Supplier for adjustment under this Clause must be asserted within twenty-eight (28) days from the date of the Supplier's receipt of the Procuring Entity's change order. 33.3 Prices to be charged by the Supplier for any Related Services that might be needed but which were not included in the Contract shall be agreed upon in advance by the parties and shall not exceed the prevailing rates charged to other parties by the Supplier for similar services.

  • Purchase Order Pricing/Product Deviation If a deviation of pricing/product on a Purchase Order or contract modification occurs between the Vendor and the TIPS Member, TIPS must be notified within five (5) business days of receipt of change order. TIPS reserves the right to terminate this agreement for cause or no cause for convenience with a thirty (30) days prior written notice. Termination for convenience is conditionally required under Federal Regulations 2 CFR part 200 if the customer is using federal funds for the procurement. All purchase orders presented to the Vendor, but not fulfilled by the Vendor, by a TIPS Member prior to the actual termination of this agreement shall be honored at the option of the TIPS Member. The awarded Vendor may terminate the agreement with ninety (90) days prior written notice to TIPS 0000 XX Xxx Xxxxx, Xxxxxxxxx, Xxxxx 00000. The vendor will be paid for goods and services delivered prior to the termination provided that the goods and services were delivered in accordance with the terms and conditions of the terminated agreement. This termination clause does not affect the sales agreements executed by the Vendor and the TIPS Member customer pursuant to this agreement. TIPS Members may negotiate a termination for convenience clause that meets the needs of the transaction based on applicable factors, such as funding sources or other needs. Usually, purchase orders or their equal are issued by participating TIPS Member to the awarded vendor and should indicate on the order that the purchase is per the applicable TIPS Agreement Number. Orders are typically emailed to TIPS at xxxxxx@xxxx-xxx.xxx. • Awarded Vendor delivers goods/services directly to the participating member. • Awarded Vendor invoices the participating TIPS Member directly. • Awarded Vendor receives payment directly from the participating member. • Fees are due to TIPS upon payment by the Member to the Vendor. Vendor agrees to pay the participation fee to TIPS for all Agreement sales upon receipt of payment including partial payment, from the Member Entity or as otherwise agreed by TIPS in writing and signed by an authorized signatory of TIPS.
