Changes in Location. The Network Service Provider shall notify the Managing Entity’s Network Manager, in writing, at least ten (10) calendar days prior to any changes in locations where services are being provided. Changes must continue to meet the service needs of consumers without excessive time and travel requirements.

Changes in Location. (1) The Managing Entity shall notify the Department’s Contract Manager, in writing, at least 10 calendar days prior to any changes in locations where services are being provided. (2) The Managing Entity shall notify the Department in writing a minimum of 30 days prior to making changes in location that will affect the Department’s ability to contact the Managing Entity by telephone or facsimile transmission.
