Confirming your booking. Your booking becomes confirmed, on the basis of these terms and conditions, upon receipt by the University of an unaltered Booking Contract signed by the Client and receipt of a 10% deposit payment. The University reserves the right to charge up to 50% deposit for bookings with significant operational implications. The Booking Contract will state the accommodation, meals and other facilities you have booked, and it will include any other arrangements which have been agreed between the Client and the University. When you confirm a booking, the Client agrees to pay all charges for accommodation, meals and other facilities set out in the Booking Contract, unless these are changed or cancelled under the terms outlined in clauses 7, 8 or 10 below. In that case the Client must pay the charges in accordance with the relevant clause.

Confirming your booking. Your booking becomes confirmed, on the basis of these terms and conditions, upon receipt of payment by the University. The Booking Confirmation will state the accommodation, meals and other facilities you have booked. The Client agrees to pay all charges for accommodation, meals and other facilities set out in the Booking Confirmation, unless these are changed or cancelled under the terms outlined below.
Confirming your booking. When the Booking has been confirmed, the Hirer agrees to pay all the charges that are listed in the Booking Contract when they are due. Where appropriate, the Hirer shall confirm the minimum and maximum numbers for the Booking. The minimum numbers will be the minimum for which the Hirer will be invoiced. Catering numbers and requirements (menus, timings etc.) must be advised to the relevant University department at the time of the booking and will be identified in the Booking Contract. Specific details to include timings, menus, numbers and special requests must be confirmed to the University department at least 14 days prior to the Event, with final numbers expected 7 working days in advance of the Event. In the event that these details are not confirmed, the University will charge on basis of the last numbers provided or the number of Guests it serves, whichever is the greater. Any amendments to these arrangements must be received, in writing, by the catering department no later than 7 working days prior to the Event. These will be the minimum number for which the Hirer will be charged. Late cancellations/reductions in numbers/amendments will be dealt with under the cancellation policy outlined in Clause 5.10.

Confirming your booking. All bookings will be regarded as provisional until a confirmation letter or email for the required services has been received, duly signed by you. By confirming your booking, you are accepting these terms and conditions. The University reserves the right to cancel the provisional reservation if written confirmation has not been received within 14 days of the provisional booking.

Confirming your booking. Your booking is officially confirmed based on the following terms and conditions, upon the receipt of either an email confirmation or a signed Booking Form by CBGC from you. The Booking Form will comprehensively outline all the facilities You have reserved, along with a clear description of any additional arrangements that have been mutually agreed upon between you and CBGC. To ensure a smooth booking process, kindly provide CBGC with confirmation of specific requirements for your event, including what we will or will not provide you on the day. This will be documented in the form of a checklist, compiled based on the details you have provided. Prior to your event, CBGC will send this checklist arriving to your inbox, allowing ample time for your review. We kindly request that you thoroughly review this checklist and make any necessary amendments if required. Your final approval, signifying the accuracy of the information, is kindly requested to be sent back to us via email within 3 days leading up to the event. In the event that you verbally request CBGC to confirm a booking, it is essential that you promptly follow up by sending an email to CBGC to reiterate and confirm your specific requirements. CBGC must receive this email within a maximum of 3 days from your initial verbal request. Failure to do so may, at the sole discretion of CBGC, result in the cancellation of the booking. By confirming a booking, you are explicitly agreeing to assume responsibility for all charges as detailed in the Booking Contract, unless these charges are subsequently modified or cancelled in accordance with the stipulations outlined in clauses 6, 7, 9, or 10 below. In such cases, you are obligated to settle the charges as specified within the respective clause.

  • Using Your Card You understand that the use of your credit card or credit card account will constitute acknowledgement of receipt and agreement to the terms of the Credit Card Agreement and Credit Card Account Opening Disclosure (Disclosure). You may use your card to make purchases from merchants and others who accept your card. The credit union is not responsible for the refusal of any merchant or financial institution to honor your card. If you wish to pay for goods or services over the Internet, you may be required to provide card number security information before you will be permitted to complete the transaction. In addition, you may obtain cash advances from the Credit Union, from other financial institutions that accept your card, and from some automated teller machines (ATMs). (Not all ATMs accept your card.) If the credit union authorizes ATM transactions with your card, it will issue you a personal identification number (PIN). To obtain cash advances from an ATM, you must use the PIN issued to you for use with your card. You agree that you will not use your card for any transaction that is illegal under applicable federal, state, or local law. Even if you use your card for an illegal transaction, you will be responsible for all amounts and charges incurred in connection with the transaction. If you are permitted to obtain cash advances on your account, you may also use your card to purchase instruments and engage in transactions that we consider the equivalent of cash. Such transactions will be posted to your account as cash advances and include, but are not limited to, wire transfers, money orders, bets, lottery tickets, and casino gaming chips, as applicable. This paragraph shall not be interpreted as permitting or authorizing any transaction that is illegal.

  • When Your Coverage Ends Coverage under this plan is guaranteed renewable. It can only be canceled by us for the following reasons: • if you leave your place of employment; • if you decide to discontinue coverage. Inform your employer prior to the requested date of cancellation and your employer will notify us. If we do not receive your notice prior to the requested date of cancellation, you or your employer may be responsible for paying another month’s premium; • if the required premium is not paid within one month of the due date. We will mail you a notice of discontinuance along with information about enrolling in an individual healthcare plan; • if you or a covered dependent no longer qualifies as an eligible person; • if we no longer offer this type of coverage; • if your employer contracts with another insurer or entity to provide or administer benefits for the covered healthcare services provided by this agreement; • if fraud is determined by us. See Rescission of Coverage section below for additional details; If your healthcare coverage is terminated for one of the reasons listed above, we will send you a termination notice thirty (30) days before the termination date. The notice will indicate the reason why your healthcare coverage has ended. When your coverage ends, you may apply for individual healthcare coverage directly from BCBSRI or through HSRI. You must meet the eligibility requirements and we must receive required enrollment information within sixty (60) days from the date your group coverage ended along with required premium. If you do not reside in Rhode Island, you are not eligible to enroll in an individual plan from BCBSRI or HSRI. You may be able to obtain coverage through an insurance company in the state in which you reside. Rescission is a cancellation or discontinuance of coverage that has a retroactive effect. A cancellation is not a rescission if it: • only has a prospective effect (as described above); or • is due to non-payment of premiums, which can have a retroactive cancellation effect. We may rescind your coverage if you or your dependents commit fraud. Fraud includes, but is not limited to, intentional misuse of your identification card (ID card) or intentional misrepresentation of a material fact. Any benefit paid in the past will be voided. You will be responsible to reimburse us for all costs and claims paid by us. We must provide you a written notice of a rescission at least thirty (30) days in advance. Except for non-payment, we will not contest this policy after it has been in force for a period of two (2) years from the later of the effective date of this agreement or the latest reinstatement date.

  • What To Do If You Find A Mistake On Your Statement If you think there is an error on your statement, write to us at the address(es) listed on your statement. In your letter, give us the following information:

(b) Since January 1, 2018 through the date hereof, the Company has timely filed with the SEC (subject to extensions pursuant to Exchange Act Rule 12b-25) each report (including each report on Forms 8-K, 10-Q and 10-K), statement (including proxy statement), schedule, exhibit, form or other document or filing required by Applicable Law to be filed by the Company at or prior to the time so required, including all certificates required pursuant to the Xxxxxxxx-Xxxxx Act. No Subsidiary of the Company is required to file or furnish any report, statement, schedule, exhibit, form, certificate or other document with the SEC. (c) As of its filing date (or, if amended or superseded by a filing prior to the date hereof, on the date of such filing), each Company SEC Document complied as to form in all material respects with the applicable requirements of the Securities Act, the Exchange Act and the Xxxxxxxx-Xxxxx Act and all other Applicable Law. (d) As of its filing date (or, if amended or superseded by a filing prior to the date hereof, on the date of such filing), no Company SEC Document filed pursuant to the Exchange Act contained any untrue statement of a material fact or omitted to state any material fact necessary in order to make the statements made therein, in light of the circumstances under which they were made, not misleading. No Company SEC Document that is a registration statement, as amended or supplemented, if applicable, filed pursuant to the Securities Act, as of the date such registration statement or amendment became effective, contained any untrue statement of a material fact or omitted to state any material fact required to be stated therein or necessary to make the statements therein not misleading. (e) The Company has Made Available to Parent correct and complete copies of all comment letters received by the Company from the SEC relating to the Company SEC Documents since January 1, 2019, together with all written responses of the Company thereto. Since the date of the Company’s annual report on Form 10-K for its fiscal year ended December 31, 2019 and as of the date hereof, (i) there are no outstanding or unresolved comments received by the Company from the SEC that would be required to be disclosed under Item 1B of Form 10-K under the Exchange Act, and (ii) to the Knowledge of the Company, none of the Company SEC Documents is the subject of any ongoing investigation by the SEC. (f) Each required form, report and document containing financial statements that has been filed with or furnished to the SEC by the Company since January 1, 2018 through the date hereof was accompanied by the certifications required to be filed or submitted by the Company’s principal executive officer and principal financial officer, as applicable, pursuant to the Xxxxxxxx-Xxxxx Act and, at the time of filing or submission of each such certification, such certification was true and accurate and complied with the Xxxxxxxx-Xxxxx Act. For purposes of this Section 4.07, “principal executive officer” and “principal financial officer” shall have the meanings given to such terms in the Xxxxxxxx-Xxxxx Act. Neither the Company, any current executive officer nor, to the Knowledge of the Company, any former executive officer of the Company, has received written notice from any Governmental Authority challenging or questioning the accuracy, completeness, form or manner of filing of such certifications made with respect to the Company SEC Documents filed prior to the date hereof. Neither the Company nor any of its Subsidiaries has outstanding (nor has arranged or modified since the enactment of the Xxxxxxxx-Xxxxx Act) any “extensions of credit” (within the meaning of Section 402 of the Xxxxxxxx-Xxxxx Act) to directors or executive officers (as defined in Rule 3b-7 under the Exchange Act) of the Company or any of its Subsidiaries. The Company is otherwise in compliance with all applicable provisions of the Xxxxxxxx-Xxxxx Act and the applicable listing and corporate governance rules of Nasdaq, in each case in all material respects.

If you or your dependents fail to enroll at this time, you cannot enroll in the plan unless you do so through an Open Enrollment Period or a Special Enrollment Period.
