Continued Coverage - Illness. It is understood in cases where employees with at least one (1) year service who are absent due to a bona fide illness or injury, confirmed by a medical practitioner, that the Employer’s responsibility for benefit coverage will be limited to twelve months. This may be waived by mutual agreement.

  • Disability Coverage In the event a State employee goes on an extended medical disability, or is receiving Workers’ Compensation benefits, the Employer-policyholder shall continue at no cost to the employee the coverage of the group life insurance for such employee for the period of such extended leave, but not beyond two (2) years.

  • Long Term Disability Insurance 250. The City, at its own cost, shall provide to employees a Long Term Disability (LTD) benefit that provides, after a one hundred and eighty (180) day elimination period, sixty percent salary (60%) (subject to integration) up to age sixty-five (65). Employees who are receiving or who are eligible to receive LTD shall be eligible to participate in the City's Catastrophic Illness Program as set forth in the ordinance governing such program.

  • Family Illness The start of a family leave for a serious health condition of a family member shall begin on the date requested by the employee or designated by Management.

  • Extended Illness Leave 1. Extended Illness Leave, hereinafter referred to as “EIL” shall be established for Administrative Support Group, XXXX, Clerical, and Educational Assistant personnel who are absent due to their own personal illness or injury following the exhaustion of their accumulated illness, vacation and annual allotment of short term leave. Eligibility for Extended Illness Leave includes completion of employee’s probationary period.
