Contraceptives. FDA approved contraceptive methods, including certain contraceptive products, sterilization procedures for women, and patient education and counseling for all women with reproductive capacity; and
Contraceptives. The Foundation will not invest in companies that manufacture contraceptives or derive more than 10% of their revenue from the sale of contraceptives, even if they do not manufacture them.
Contraceptives. We cover prescription contraceptives which require a Practitioner’s prescription and which are approved by the United States Food and Drug Administration for that purpose. In addition, We cover over-the-counter contraceptive drugs which are approved by the United States Food and Drug Administration for that purpose without a prescription.
a) We cover the following services, drugs, devices and procedures when obtained from or provided by network providers:
1. Contraceptive drugs, devices or products approved by the United States Food and Drug Administration; or
2. Therapeutic equivalents of contraceptive drugs, devices or products that are approved by the United States Food and Drug Administration.
3. The medical necessity for contraceptive drugs, devices or products shall be as determined by the [Member]’s Practitioner.
b) Voluntary sterilization of a [Member] whether male or female;
c) Patient education and counseling on contraception for a [Member];
d) Services related to the administration and monitoring of drugs, devices, products and services covered under this Contraceptives provision, including, but not limited to:
1. Management of side effects;
2. Counseling for continued adherence to a prescribed regimen;
3. Device insertion and removal;
4. Coverage of alternative contraceptive drugs, devices or products the Member’s practitioner determines are medically necessary; and
5. Diagnosis and treatment services provided pursuant to or as a follow-up to services covered under this Contraceptives provision. Coverage is provided for a twelve-month period. [Except as stated below] Coverage under this Contraceptives provision is provided without the application of any deductible, coinsurance or copayment. Exception: With respect to a male [Member], coverage of male sterilization and male contraceptives is subject to the minimum deductible permitted for a high deductible health plan under section 223(c)(2)(A) of the Internal Revenue Code. The deductible for this coverage is stated in the Schedule.
Contraceptives. The following contraceptives and contraceptive devices are covered upon prescription or upon the Participating Physician's order only at a Participating Retail or Mail Order Pharmacy:
Contraceptives. Unless otherwise covered under a prescription drug program, Benefits for contraceptives include those contraceptive products or devices mandated by PPACA including but not limited to contraceptive implants such as intrauterine devices (IUD) and services related to the fitting, insertion, implantation and removal of such devices.
Contraceptives. Consultations, examinations, procedures and medical services provided on an outpatient basis and related to the use of contraceptive methods are covered. Coverage includes the Participating Physician’s insertion and removal and Medically Necessary examination associated with the use of contraceptive drugs or devices approved by the Food and Drug Administration (FDA). An office visit Copayment will apply, if any. Coverage is not provided if the Contract Holder is a religious organization who has elected not to provide coverage for contraceptive services or treatment.
