DECLARATION OF INTERESTS. The contractor will provide written formal declaration of interests from all of the management and project team members involved in the production of the evidence review. A standard form will be provided by the NICE team.
DECLARATION OF INTERESTS. (1) Each Commissioner, other than the President, and each Deputy Commissioner shall disclose to the President any direct or indirect pecuniary interest held or acquired by that person in a business carried on in Australia or in a body corporate carrying on such a business that could conflict with that person's duties as a Commissioner or Deputy Commissioner.
(2) The President shall give written notice to the Chairperson of the Ministerial Council of all direct and indirect pecuniary interests held or acquired by the President in any business carried on in Australia or in any body corporate carrying on any such business.

DECLARATION OF INTERESTS. 11.1 Before commencing work for us (and annually thereafter on our request), you must complete our Declaration of Competing Interests Form and send it to us. We will provide a link to access this form following your acceptance of this agreement. You must notify us immediately if, at any time during your appointment, any new (undeclared) conflict of interest arises or the nature of any existing (previously declared) conflict of interest changes.
11.2 You consent to us publishing any interests you declare pursuant to clause 11.1 (or otherwise), in the Journal and on the Journal’s website.
11.3 If we determine that a conflict of interest exists in respect of your appointment, we shall have the right to immediately terminate your appointment pursuant to clause 4.2.5 of these Terms.
DECLARATION OF INTERESTS. Councillors shall abide by the Members' Code of Conduct. It is the responsibility of every Councillor to declare an interest by the time the relevant item of business is reached.

It is a requirement that all Joint Committee members should declare any personal or business interests they may have which may affect, or be perceived to affect the conduct of their role as a Joint Committee member. This includes any interests that may influence or be perceived to influence their judgement in the course of conducting the Joint Committee’s business. Joint Committee members must be familiar with the Values and Standards of Behaviour Framework and their statutory duties under the relevant Constitution Regulations. Joint Committee members must notify the Joint Committee of any such interests at the time of their appointment, and any further interests as they arise throughout their tenure as Joint Committee members.

DECLARATION OF INTERESTS. If WHO considers that it needs to rely on the Consultant's independence and objectivity in the performance of his/her work under this contract, WHO shall require the Consultant to complete a WHO Declaration of Interest ("DoI") and disclose any circumstances that could give rise to a potential conflict of interest in relation to the work. In such event, the DoI Form must be completed by the Consultant and evaluated by WHO, before the contract can become effective. Thus, the Consultant undertakes not to commence work on the contract, until WHO has confirmed to the Consultant in writing that the information disclosed by the Consultant does not require modification or cancellation of the contract.
DECLARATION OF INTERESTS. 5.12.1 The agenda for a meeting of the SLP Board will include a standing item for the Declaration of Interests in respect of any agenda item where a decision is sought.
5.12.2 The SLP Board will decide whether the member with a potential financial or non-financial interest is allowed to participate in the discussion, observe the discussion, or whether it would be appropriate for them to play no part by leaving the room. It is for the SLP Board meeting to determine the most appropriate action, dependent on the extent of the conflict and the scale of the financial implications to the organisation/individual involved.
5.12.3 Where applicable, Members shall have regard to their respective Officer Code of Conduct.
DECLARATION OF INTERESTS. 24.1 In order to ensure the highest standards of probity, the Service Provider must inform the Council in writing of any Member or employee of the Council who is or becomes involved in the Service Provider's undertaking at any time that this Contract is in operation.
