Discharge Planning may occur only once within thirty (30) calendar days prior to the date of the last face-to-face treatment with the Client. The Discharge/Exit Plan is developed during this session and shall include referrals to outside entities to ensure recovery/whole person care as needed. It shall also list relapse triggers and a plan to deal with each to prevent relapse. A copy shall be provided to the Client.

Discharge Planning Services. CONTRACTOR shall prepare, and provide each client and COUNTY with, written discharge and relapse prevention plans, which include, without limitation, adequate housing, employment opportunities and/or referrals, a realistic budget, drug free personal support system and appropriate recreational activities. Discharge planning services shall begin on the Member's admission and shall be completed by the medically appropriate discharge date. Upon request, PROVIDER shall provide documentation supporting the progress of discharge planning services any time during a Member's stay at Facility.

  • Training Services Training Services may include pre-packaged training Products, and/or the development or customization of training programs as requested, including Live Training, Computer Based/Multi-Media Training which encompasses Internet-Delivered Training, and/or Video Based Training.

  • Client Responsibilities You are responsible for (a) assessing each participants’ suitability for the Training, (b) enrollment in the appropriate course(s) and (c) your participants’ attendance at scheduled courses.

  • Security Services CONTRACTOR shall provide security services designed for preventing escapes, maintaining order, providing care, custody, control, supervision and management of the inmate population. A successful security program depends heavily on staff training, effective administration, and the establishment of inmate programs. The organization, staffing, and administration of the security program are vital to the Facility. The direction provided by a well-organized and clearly articulated operations manual, and emergency planning provides a solid base for successful administration. The final operations manual shall be submitted to the Bureau prior to the Service Commencement Date and shall be reviewed annually and updated as needed. Documentation of the review shall be provided annually to the On- Site Contract Monitor. Changes to the plan require written permission by the Contract Manager. The Department reserves the right to require changes to plans submitted to the Bureau.
