Professional Consultation: Professional consultation is an important component of a healthy psychotherapy practice. As such, I regularly participate in clinical, ethical, and legal consultation with appropriate professionals. During such consultations, I will not reveal any personally identifying information regarding you or your situation. Collaboration with Other Professionals: In order to provide quality services, I may need to collaborate with other professionals, such as your physician, psychiatrist, neurologist, and/or other mental health professionals. You will be asked to complete a release of information authorizing these exchanges. Records and Record Keeping: I may take notes during session, and will also produce other notes and records regarding your treatment. Dual Relationships Therapy never involves sexual or any other dual relationship that impairs Therapist's objectivity, clinical judgment or can be exploitative in nature. Therapist will assess carefully carefully before entering into a non-sexual and non-exploitative dual relationship with others. It may be possible that you know some of my other Patients. You may see a friend or acquaintance in my lobby. We may see each other in the community. Therapist will never acknowledge working with anyone without his/her written permission and will always ask Patients who meet in the waiting room to respect each other's privacy as well.

There are certain situations where sharing information between your child’s school, pediatrician, or other professional may be beneficial to treatment and this will be discussed with you and you will be asked to sign a Consent for the Release of Confidential Information form. There are specific times, however, when the law requires us to give information about you or your child with or without your consent.
