For this same reason, Xxxxx Xxxxxx requests that clients not communicate with her via any interactive or social networking web sites. AUDIO OR VIDEO RECORDING: Unless otherwise agreed to by all parties beforehand, there shall be no audio or video recording of therapy sessions, phone calls, or any other services provided by Xxxxx Xxxxxx.
DUAL RELATIONSHIPS. Supervisors will not be related to the students under their supervision, nor will they have a personal relationship, which undermines the authority or effectiveness of the supervision.
DUAL RELATIONSHIPS. If you are in contact with Xx. Xxxxxxx outside the office, either personally or in business context, he will never acknowledge working therapeutically with you without your written permission. If contact outside of therapy is desired or unavoidable, he will discuss with you the potential complexities, benefits, and/or difficulties that may be involved prior to entering into the dual relationship. If the dual relationship becomes uncomfortable for you in any way, or if Xx. Xxxxxxx finds it interfering with the effectiveness of the therapeutic process or the welfare of the patient, it will be critical for you and him to discuss the situation and possibly discontinue the dual relationship.

If a situation arises where you feel your therapeutic relationship has become uncomfortable or compromised, please discuss concerns with Xx. Xxxxxx and referrals will be made or adjustments accommodated as needed. In an attempt to limit potential dual relationships or exposure of patients, Xx. Xxxx Xxxxxx will not accept client “friends” on social media. I have read the above Agreement and Office Policies and General Information carefully; I understand them and agree to comply with them:
