Employee Performance Review. (a) Two (2) evaluations of the Nurse's work will be done before the completion of the probationary period. One (1) preliminary verbal evaluation will be completed after the Nurse has completed approximately thirty (30) tours. The final (written) evaluation will be completed prior to the conclusion of the probationary period. A copy of the written evaluation, signed by the Nurse, will be retained in the Nurse's file. Written evaluations will be completed every twenty four (24) months thereafter. Each Nurse shall receive a signed copy of the evaluation. (b) When as a result of a formal review of a Nurse's performance, the performance of a Nurse is judged to have been unsatisfactory, the Nurse concerned must be given an opportunity to sign and make written comment on the review form in question, to indicate that its contents have been read and explained.
