Employee Sign-on Form. The Employee shall provide a completed Employee Sign-on Form, included as Appendix “D” to this Agreement, to the Employer before commencing work. Employee Termination Record In all cases of termination an Employee Termination Record, in the form of Appendix “E”, which includes the hours worked by the Employee in the final pay period, shall be completed and provided to the Employee to finalize his employment. The form shall be signed by both the Employee and the Employer’s supervisory authority.
