Health & Welfare Trust Fund. 20.01 For the duration of the current Collective Bargaining Agreement between the said parties, and any renewals or extensions thereof, the Employer agrees to make payments to the Ironworkers Health & Welfare Trust Fund of Western Canada for each employee covered by the said Collective Bargaining Agreement as follows: 20.02 For each hour or portion thereof for which an employee receives pay, the Employer shall make a contribution as per Article l6.00 of this Agreement to the above named Health and Welfare Trust Fund. 20.03 For the purpose of this article, hours for which pay is received by the employee, in accordance with the Collective Bargaining Agreement, shall be counted as hours for which contributions are payable and each overtime hour shall be counted as one regular hour for which contributions are payable. 20.04 Contributions shall be paid on behalf of an employee starting with the employee's first hour of employment in a job classification covered by the Collective Bargaining Agreement. 20.05 The payments to the Health and Welfare Fund required above, shall be made to the Ironworkers Health and Welfare Trust Fund of Western Canada, Funds Administrative Service, 9th floor, 0000 000 Xxxxxx, Xxxxxxxx, Xxxxxxx, X0X 0X0 established under an Agreement and Declaration of Trust which shall provide for joint Administration of an equal number of Employer and Union Trustees. When the said Trust Agreement was adopted, the Employer and the Union agreed to become parties to such an Agreement and to be bound by all the terms and provisions thereof, and a copy of such Trust Agreement shall be attached to and become part of this Collective Bargaining Agreement. 20.06 It is agreed that all contributions shall be made at such time and in such manner as the Trustees of the Health and Welfare require; and the Trustees shall have the authority to have an independent person, who is qualified to perform an audit, audit the payroll and wage records of the Employer for the purpose of determining the accuracy of contributions to the Health and Welfare Trust Fund of Western Canada. 20.07 If an Employer fails to make contributions to the Health and Welfare Fund within fifteen (15) days after the date required by the Trustees of the Health and Welfare Fund, the Trustees shall have the right to take whatever steps are necessary to secure compliance with this Article, any provisions of the Collective Bargaining Agreement to the contrary notwithstanding, and the Employer shall be...

Health & Welfare Trust Fund. 40.1 The Company agrees to have a Health and Welfare Trust Fund for all part-time employees who do not qualify for the existing Company Group Insurance benefits. Health & Welfare Trust Fund. (a) In this addendum, each Employer agrees to pay, as stipulated in Schedules A, B, C, and D for each hour worked by all employees covered by this Agreement. The contributions shall be paid to the IBEW/NECA Sound and Communications Health and Welfare Trust Fund, a jointly trusted Health and Welfare Trust Fund created pursuant to Section 302(c) of the Labor management Relations Act. Payment shall be due on the 10th day of the month in which coverage will exist subject to the provisions of (b) below. The money shall be forwarded to the Administrator of the Trust monthly. The Employer further agrees to be bound by the provisions of he Trust Agreement created by the IBEW/NECA Sound and Communications Health and Welfare Trust Fund and all amendments hereinafter adopted by the Trustees and agrees to accept as its representatives the present Employer Trustees and their lawfully appointed successors. (b) In this addendum, to become initially eligible for participation in the program, the Trust Fund must have received a minimum of 125 hours on the employee, and coverage will become effective the first of the following month. (c) In this addendum, employees who accrued vested coverage under the prior NCSCA Agreement shall retain those rights under this Agreement. (d) In this addendum, each remittance will be accompanied by a form which will be furnished for this purpose. Failure to forward negotiable remittances for the entire payment due by the tenth (10th) day of the month will automatically require immediate payment of damages prescribed by the Trust Agreement as well as delinquent amounts due and will further require other action as set forth in the Trust Agreement. (e) In this addendum, in addition to the damages assessable for late payment under the Trust Agreement and to other legal action which may be taken to collect delinquent payments which have not been received by the Board of Trustees of the IBEW/NECA Sound and Communications Health and Welfare Trust by the tenth (10th) of the month individual Employers who fail to remit in accordance with the provisions shall be subject to having this Addendum terminated upon seventy-two

Health & Welfare Trust Fund. Effective May 1, 1994 a contribution of two dol- lars and twenty-five cents ($2.25) per hour for each straight time hour, and three dollars and thirty-eight cents ($3.38) for each overtime hour at time and one-half (1- 1/2), and four dollars and fifty cents ($4.50) for each over- time hour at double time, will be paid by the Employer to the Trustees of the Bricklayers & Masons Welfare Plan. It will not be a breach of this Agreement for the Union to withdraw its members from any job when the Employer has not made the proper payments to the Welfare Plan in accordance with this Agreement. Each Trustee appointed by the Union shall be a member of the Union and employed by the Union and each Management trustee shall be a salaried executive or officer of a signatory Employer, or an employee of an Employer association, on behalf of its member employers. Section 1. The Employer agrees to pay all contributions found on the “Schedule A” for each hour worked by each employee. The Employer is bound by and to the Agreement and Declaration of Trust, amended from time to time thereafter, establishing the Employees Painters Health & Welfare Trust Section 2. All of the sums herein as noted above are required to be paid to the Trust Fund by the Employer no later than the fifteenth (15th) of the month for all hours worked in the previous month. Section 3. The Employer hereby irrevocably designates as its representatives on the Board of Trustee such Trustees as are now serving, or who will in the future serve, as employer Trustees, together with their successors. The Employer further agrees to be bound by all actions taken by the Trustees pursuant to the said Agreement and Declaration of Trust, as amended from time to time.
