Employee's Responsibilities Information. All employees covered by this Contract shall be required to furnish to the Employer such information as may be requested for the proper administration of this section. For an absence of six (6) or more consecutive days to be charged against sick leave, a certificate from a medical doctor will be required prior to the issuing of the employee's next payroll payment. When the employee requests to return to work, the doctor's certificate shall contain information on whether the employee can return to work without medical limitations with regard to the employee's performance of the duties normally associated with the job classification.
