Employer Notified. An Employee who is unable to report for work due to sickness and/or accident shall notify his/her immediate Supervisor accordingly (or cause to be notified) within one-half (½) hour after the office opens.

  • Employer Requested Leave Leave of absence without loss of pay, seniority and all benefits shall be granted to employees whenever the Employer requires an employee to take designated courses and/or examinations.   • Effective Date of Benefit Termination Medical, dental and life coverage termination will take effect on the first of the month following the loss of eligible employee or dependent status.
