Enrollment Materials. Customer shall distribute to each of its employees, and to each employee of any subsidiary or affiliate of Customer participating in the Plan who is eligible to become a Participant, a copy of the Enrollment Materials or a copy of the SESO exercise form and shall be solely responsible for collecting and delivering to Transfer Agent a new account application properly completed and executed by each Participant. Customer shall in no event permit an employee who has not attained the age of majority in the state in which the employee resides to become a Participant and shall refuse to accept Enrollment Materials completed by such an individual.

Enrollment Materials. Implementation materials, if elected by the Plan Administrator, will be provided by HEBP during the enrollment process. Any custom designed materials may be subject to Supplemental Billing.
Enrollment Materials. The STATE must approve all enrollment materials including the Evidence of Coverage (EOC) sent to Enrollees prior to their use. The MCO must submit its enrollment materials in their final version before approval from the STATE can be given. Approvals by the STATE for these materials shall not be unreasonably withheld. The STATE agrees to inform the MCO of its approval or denial within thirty (30) days of receipt of these documents from the MCO.

The Contractor shall make clear any restrictions on the enrollee’s freedom of choice among network providers.

Enrollment Materials. All enrollment agreements, forms and other materials, including Subscriber handbooks and identification cards, used for Contractholders and Subscribers shall be either PHS's or Guardian's existing approved forms, modified as necessary to reflect the terms of this Agreement. and the administrative requirements of PHS and Guardian, respectively. All such materials shall specify that PHS is the underwriter of HMO Plans, and that Guardian is the underwriter of Health Insurance Contracts and Ancillary Products. Neither party will use enrollment materials reflecting this Agreement, or using the name of the other party, without the other party's approval.
