Evidence of Insurability. Any medical evidence acceptable to the Company to enable the Member’s inclusion in the Scheme and for subsequent increases in benefit. General Conditions: The general conditions to this Policy set out on pages 7 and 8 of this document. Life Assurance Benefit: The amount of benefit appropriate to a Member at his date of death as stated in Table A, subject to the Catastrophic Event limit. Member: An Eligible Member whose membership of the Scheme has commenced. Membership Conditions: The conditions relating to Membership set out on page 9 of this document. Normal Entry Date: The date on which the Member joined the Scheme.
Evidence of Insurability. Evidence of insurability deemed satisfactory by the Insurer must be submitted under the following circumstances:
Evidence of Insurability. The Contractor shall obtain and retain insurance, including workers’ compensation, automobile insurance, medical malpractice, and general liability as applicable or as required by State or Federal law and shall provide evidence of insurability (Certificate of Insurance) from the insurance carrier prior to commencement of any work in connection with the Contract. The Contractor is also required to comply with applicable Federal and State occupational disease statutes. If occupational diseases are not covered under those statutes, they shall be covered under the employer’s section of the insurance policy. The Contractor shall timely renew the policies to be carried pursuant to this section throughout the term of the Contract and shall provide OSDH Procurement Service with evidence of such insurance and renewals. Such policy shall require thirty days advance notice of cancellation be provided to OSDH Procurement Service. If the Contractor does not carry workers’ compensation insurance because it considers their business to be that of an independent Contractor, as defined by the Workers Compensation Act (85 O.S. § 1 et. seq.), and not that of an employee, the Contractor must complete the OSDH Affidavit of Independent Contractor Status.
Evidence of Insurability. Prior to purchasing a life insurance policy, the Plan Administrator may require the individual whose life is being insured to provide evidence of insurability, such as a physical examination, as may be required by the Insurer.
Evidence of Insurability. Platform shall have the capability to initiate the Evidence of Insurability (EOI) process with respect to Guardian’s EOI electronic portal (commonly known as “EOI web application”) and ensure that the appropriate rules which trigger EOI events are added, when applicable. Platform shall also have the capability to retrieve decisions provided electronically by Guardian and update pending Plan coverage statuses following the completion of the Evidence of Insurability process.

AMG shall have received such evidence as it shall deem necessary or appropriate as to the insurability of each of the Persons listed in Schedule 3.25(b) hereto as and in amounts contemplated by Section 3.5(e) of the Restated LLC Agreement with respect to both key-man life insurance and disability insurance policies.
