For Priority SYV Activities. Your Activity must support the development of well-researched and tailored services for the younger veterans’ community with a particular focus on services supporting those ‘at risk’ of experiencing poor mental health. Your Activity must: • Be based on research into particular aspect of mental health and social well-being issue/s experienced by younger veterans, as they transition to civilian life. • Develop and implement service delivery model/s to address the identified issue/s. • Include measureable outcomes around the extent to which the funded activity addressed the identified issue/s and include assessment of why it was successful/unsuccessful. • Provide a report of those outcomes at the conclusion of the Activity. For standard SYV Activities: Your Activity must provide one or more of the following outcomes for the veteran community: • Deliver services to the younger veterans’ community that are innovative, sustainable, build community capacity for the future and complement services already provided by the Commonwealth. • Expand on existing or well established services offered to the younger veterans’ community to new regions within Australia or in development of new services that will be offered in more than one location. • Raise awareness of the important issues faced by the younger veterans’ community. You must: Activity details. The Activity must align with your proposal in your relevant application and the approved funding may be used for: Approved funded item(s). Data Exchange Reporting You are required to provide client level data and service delivery information from all recipients of this Activity in accordance with the Data Exchange Protocols (xxxxx://xxx.xxx.xxx.xx/xxxx-xxxxxxxx-xxxxxxxxx/). You must provide the data required within the Data Exchange through an approved mechanism as outlined in the Data Exchange Protocols. You are required to finalise the submission of data within the Data Exchange for each reporting period within 30 days of the reporting period ceasing, as set out in the reporting schedule below. For this Activity, participation in the “partnership approach” is a requirement of funding. By participating, you agree to provide some additional information in exchange for the receipt of regular and relevant reports.
