Contract Form definition

Contract Form means the prototype of a "contract" and any associated riders and endorsements filed with the commissioner by a health care service contractor or health maintenance organization.
Contract Form has the meaning given to it in Section 1 above; "Contract Period" means the period commencing from the Commencement Date and ending on the End Date, where the context so requires;

Contract Form means the form setting out the principal terms of a Contract (save for a purchase order), as agreed between the Company and the Supplier in accordance with these Conditions.
