The Initial Assessments clause establishes the requirement for an early evaluation or review at the outset of an agreement or project. Typically, this involves one or both parties conducting a preliminary analysis of relevant factors, such as project feasibility, risks, or compliance with certain standards, before proceeding further. By mandating this early assessment, the clause helps identify potential issues or obstacles in advance, ensuring that both parties are informed and can make decisions or adjustments before significant resources are committed.
Initial Assessments. To enable us to establish confidence that your management system(s) meet the requirements of this agreement and the specified standard, we require you to: • Allow us full access to all work areas, personnel, records, and documentation relevant to your application. • Demonstrate compliance to the specified standard, to our satisfaction. • Nominate a management representative and advise us of this nomination. If we are not satisfied that compliance can be adequately demonstrated during the initial assessment, it may be necessary to undertake further special visits. Should this be the case, they will be arranged in advance, and a written quotation provided.
Initial Assessments. Initial IEPs for students shall be assigned on a rotating basis among all the Blended Model Special Education teachers at that site.
Initial Assessments. 21.14.5.1 Beginning with the 2015-2016 school year, in order to make RS assignments in a manner intended to achieve a reasonable balance among RS’s, the number of initial assessments performed by each RS should not exceed 12 annually.
21.14.5.2 When an individual RS approaches 12 assigned initial assessments, the following steps will occur:
21.14.5.2.1 The District shall exhaust all available options to prevent the assignment of additional initial assessments that would cause the number in 21.14.5.1 to be exceeded.
21.14.5.2.2 The Special Education Committee (see Article 16.3) shall monitor initial assessments of all RS’s on at least a monthly basis to achieve the number in 21.14.5.1.
Initial Assessments. All clients who are admitted to the SRF will undergo the following assessments:
3.2.1. Physical examination and physician’s report completed before admission
3.2.2. Activities assessment by the SRF Program Director during admission
3.2.3. Initial evaluation/care and medication plan completed by a licensed nurse during admission
3.2.4. Initial mental health evaluation/assessment completed and signed by a licensed mental health professional during the time of admission. Within 72 hours of admission, the Program Director in collaboration with the client, client’s representative, family (where applicable), licensed nurse, and licensed mental health professional will develop an Individual Treatment Plan for the client. This Plan will be updated every 60 days or as needed.
Initial Assessments. HMOs shall conduct a comprehensive assessment for each member within 60 days of enrollment in the HMO.
a) The HMO must perform a comprehensive assessment for a member that was previously enrolled in the HMO but re-enrolls in the HMO at least six months after his/her last disenrollment.
b) The assessment shall be conducted by a care coordinator or an appropriately qualified health care professional either through face-to-face contact or telephonic contact with the member and/or legal guardian.
c) The assessment process shall be comprehensive and be consistent with the following principles: • Identify the member’s recovery goals and understanding of options for treatment. • Address the strengths, needs, recovery goals, priorities, preferences, values and lifestyle described by the member. • Identify the cultural and environmental supports of the member as they affect identified goals and desired outcomes and preferred methods for achieving the identified goals. • Be updated as new information becomes available or with a change in the member’s condition.
d) The assessment shall include the following elements to be considered “comprehensive”: • Diagnosis and health related services. • Mental health and substance use. • Demographic information (including ethnicity, education, living situation/housing, legal status). • Activities of daily living (including bathing, dressing, and eating). • Instrumental activities of daily living (including medication management, money management, and transportation). • Overnight care and employment. • Communication and cognition (ability to communicate memory). • Indirect supports (family, social, and community network). • General health and life goals. • Any other health-related domain identified by the Department.
Initial Assessments. 1. All initial assessments shall be performed by staff who are trained and privileged in performing assessments of behavioral health disorders. Additional, for Title XIX and Title XXI members, the clinician must, at minimum, be a behavioral health professional.
2. For members referred for or identified as needing on going psychotropic medications for a behavioral health condition, the Contractor shall ensure the review of the initial assessment and treatment recommendation by a licensed medical professional with prescribing privileges.
3. The initial assessment shall include recommendations for services, including case management.
4. The Contractor shall develop and implement privileging criteria that are inclusive of age-and population-specific competencies, subject to RBHA and ADHS approval, for the performance of initial assessments.
5. Upon completion of a thorough competency assessment of each individual who will perform initial assessments, the Contractor shall submit an Attestation of Privileges for each individual deemed competent.
6. The Contractor shall comply with all RBHA initiatives to evaluate the capacity of the system of care with regard to availability of staff within the provider network eligible to provide intake services including, but not limited to:
Initial Assessments. It is recommended to seek out a free Initial Assessment through Canon. This assessment will provide a detailed analysis of the agency’s current print status and an estimated savings with a phased implementation recommendation. If an agency elects to not utilize this assessment, their IT group should be contacted for assistance. The contract administrator is also available to provide guidance. NOTE: There have been issues with Agencies ordering 6000 and 5000 series devices which require a NEMA plug. When ordering, please discuss any special needs with Canon. The Initial Assessment would bring any unusual requirements to the Agency’s attention and is highly recommended.
Initial Assessments. Upon referral, Provider will conduct an initial assessment to determine the suitability of play therapy services for the client's needs. There are specific forms that must to be completed as a prerequisite when referring a child for play therapy. This includes: • Parent/ Carer Intake Assessment Form; • Strength and Difficulties Questionnaire (SDQs); • Parent/Carer Consent Form; • Goals Based Outcomes (GBO). On completion of these forms, I will make a recommendation for play therapy based on the assessment that I have completed. SDQS and GBO forms are to be completed on a termly basis to assess the progress that the child is making.
Initial Assessments. (A) The secretary of administration shall make an initial assessment in the amount of $12,500,000.00 within 30 days following ratification of the compact agreement by the Congress of the United States. The amount assessed shall be paid within 15 days of the assessment.
